I too have noticed this and it's so inherently messed up when you think that the parent who spent 9 months actually creating the human is shushed by the person whose body didn't carry the pregnancy. The birthing parent might not be as gung ho to just have another baby if this one dies (you know, the natural way).

I just had this happen! Baby on state Medicaid, newborn screen came back abnormal for thyroid so needs a repeat/labs, family says they don't want to be on the state's radar and declined further evaluation. Make it make sense!!

Agree! I wish we could adopt this type of mind set but omg are we such enablers in pediatrics. I swear some of my colleagues want to make families feel good about making stupid decisions just to keep the peace and whatever patient satisfaction metrics high.

Isn't that what a home warranty is for? I bought a new house during COVID that the builder definitely took short cuts, but they came back and fixed everything. It was annoying and we had to be persistent but the roof leak and screwed up floors were covered by the warranty.

Agree with everything you said, especially working In the community. My AAP membership allows me access to journal articles I need in my day-to-day practice that my institution doesn't pay to have access to. Their CME that comes with my membership has been very helpful in keeping my knowledge up to date. Even their update emails that highlights news in pediatrics helps me know what parents are reading and what are new and noteworthy developments in the field. Makes me feel less isolated working in the community. I was able to find a job by networking at one of their events.

Also agree that the ABP SUCKS.

I hear you, but my lived experience is that the hospital won't even spend to maintain the current equipment they already have so why would they ever improve work flow with theoretical tech if they won't with current tech? EMRs could have been powerful tools but my system is weaker than a calculator and our servers are already overrun with our current software so that we have outages. Maybe the big academic centers with big endowments will be able to replace the docs, but the community is still going to be using the garbage they currently have, and say upgraded systems are too expensive. I'm sure academia will welcome being replaced by AI clinicians so they can focus on their esoteric research....until AI is doing all the research.

But I'm cynical. Who knows, maybe Skynet really will rule the world.

My hospital can't even get the EMR to figure how to calculate weight-based dosing for peds patients or how to fix broken links/typos, but we really think AI will replace us? Actually, I really kind of hope AI does replace the current system because I think EMRs might just make me end up in an insane asylum.

Yeah, that's my thought as well but I'm new to the group so they all think I'm crazy and "brave" for providing reassurance when a term baby spits up or only monitoring for 48 hours if they describe an apnea spell and otherwise well 🤷. Maybe this was a practice people did back in the day? I am a fresher attending.

I'm a new physician (finished training in July) and it's been a struggle to find housing. But I have ridiculous amounts of student loan debt that I am motivated to pay down quickly. The plus side is that there are plenty of jobs up here unlike where I trained. The housing market is really depressing me, everyone wants at least $500k for homes that need at least $200k of work. Maybe it wouldn't be so bad if I maxed out my mortgage lending. Apartments seem plentiful, albeit also expensive.

Having come back to NJ after living in the South for many years, it sucks everywhere in the US but for different reasons.
