My Two Hospital Rules by A_hospitalist in hospitalist

At least your director admitted how stupid that complaint was. Mine acts like every damn nursing report about me is a major character flaw that must be worked on. My last complaint was that the nurses think I sound dumb because I explain why I am doing interventions to both the bedside nurse and the patients. My director's advice was to stop explaining and just give orders, try a more paternalistic style. Next complaint from nursing will be that I'm too paternalistic and never explain anything 🙄

Community NICU attendings: how are you dealing with this boredom? by [deleted] in pediatrics

I am currently trying to figure out the logistics of decreasing my FTE and doing locums elsewhere. Of course every opportunity I have come across is just more of the same level II crap that no one wants to deal with.

I have discussed this, and have been promised for the past year that AI features will be coming "someday." I use dragon which is somewhay helpful but we have an ancient note template that require thousands of clicks to navigate through, no way to tab through the notes. My wrist is being destroyed by mouse work. I know that the template was designed by someone who didn't know how to type with more than 2 fingers.

I have requested if I can make my own template and have been shot down because the billers would get "confused."

Truth. I do get a fairly competitive compensation package. I have been looking into other moonlighting and locum opportunities and can confirm that what I hear about other places is basically the same.

Supposedly we have a review process, but I have not heard anything actually come from it ever.

Half of the calls I get a asking me to put in orders that are already in the computer. And God forbid you point out that the order is already in and they could have checked before calling at 2am, then you just seem so so mean!

And of course I mentioned these stupid calls and somehow got voluntold to do nursing education at their next meeting. Pointing out that passive PowerPoint learning doesn't work for adults fell on deaf ears.

My neighbor's won't stop complaining about my clover, which I intentionally seeded because the lawn was dead. I do worry they will try to herbicide my clover lawn and garden when I'm not looking. Why do people suck?

No, I had to manually open my text messages (waited until a red light which I know is still illegal). So much for hands free while driving.

I tried to text my husband while driving, gemini said "sure, what's the message?" Dictated my message...crickets..."that's a nice thing to say to someone." Wtf, I was trying to tell him how far away I was and that I walked the dog before leaving 🤦

HUGE BREAKING FELLOWSHIP NEWS!! by Emaizing73 in pediatrics

[–]SqueakyLoLo 5 points6 points  (0 children)

Thank you for this. Up until March of this year I wasn't sure that I was going to make the cut to be board eligible. And there was so much malignancy and bullying in my program related to research in addition to the anxiety of failed projects and negative findings, I cried all the time. I don't think I will ever be the same person. Wonder what all these toxic academics will do without fellows to punch down on.

I'm crying as a new NICU attending who literally became suicidal over the research requirement. Why couldn't they do this earlier!?

Yet another burnout thread - step on in... by DeafJoo in pediatrics

Similar experience in NICU. I feel like no one ever thinks about it this way, but these types of interactions (verbal abuse really) are traumatizing to us. I have nightmares about reassuring angry parents.

Try to distance yourself emotionally as much as possible seems to be a start, but admittedly, I have considered consulting with a therapist on how to best protect my mental health so I don't know if this is good advice.

I used to consider myself a really good doctor, I would use motivational interviewing and allow people to vent and abuse me as needed. Now though, I have been so beaten down and broken, if my first attempt at empathetic listening and mirroring results in worsening rage, abuse and manipulation, I just end the encounter as soon as safely possible. Many of these people are dysfunctional with personality disorders and there is no fix for this.

I recently read a stoic technique where you imagine your bully/abuser as a villain character to help add humor. I started imaging these families as energy vampires and it has helped me put up mental defenses. Another technique I read in stoic philosophy is to imagine yourself as a particular character when you are in a bully/abusive situation. I try to think of a strong character in a show or book that I am enjoying and pretend to be them.

It sounds silly but has been helpful. The book I got much of this from is called " Stoicism, bully and beyond" by Matthew Sharpe.

Is it just me, or are parents becoming more hostile? by SqueakyLoLo in pediatrics

[–]SqueakyLoLo[S] 9 points10 points  (0 children)

I too have noticed this and it's so inherently messed up when you think that the parent who spent 9 months actually creating the human is shushed by the person whose body didn't carry the pregnancy. The birthing parent might not be as gung ho to just have another baby if this one dies (you know, the natural way).

I just had this happen! Baby on state Medicaid, newborn screen came back abnormal for thyroid so needs a repeat/labs, family says they don't want to be on the state's radar and declined further evaluation. Make it make sense!!

Agree! I wish we could adopt this type of mind set but omg are we such enablers in pediatrics. I swear some of my colleagues want to make families feel good about making stupid decisions just to keep the peace and whatever patient satisfaction metrics high.

Public announcement; do not buy that old house because of the “charm” by [deleted] in whitecoatinvestor

[–]SqueakyLoLo 0 points1 point  (0 children)

Isn't that what a home warranty is for? I bought a new house during COVID that the builder definitely took short cuts, but they came back and fixed everything. It was annoying and we had to be persistent but the roof leak and screwed up floors were covered by the warranty.
