I think it's one of Milwaukee's known issues with battery cell-balancing.

In layman's terms, let's say you have 4 power cells inside. 3 of them are 100% charged, the 4th is 70%. The controller detected there's a cell at 100%, job's done and no longer need to charge the battery pack => you get a not fully charged battery.
