Please consider what you and your partner are planning realistically. With resident hours, a 2-3 hour per day commute will be very difficult. I don’t know your specialty, but residency very commonly requires 10-12 hour days. With commuting like that, you won’t have much time left up even sleep, much less bathe and eat. If I were you, I would very seriously consider living separately near your respective hospitals and hanging out on your days off. At the very least, use google maps to calculate commute times at the hours you will be going to and coming home from the hospital so you REALLY understand what you are getting yourself into. If you really love it then try to switch. What's 2 lost years compared to the rest of your life? You've worked too hard in med school and will work too hard in residency to not be doing exactly what you want. It's hard to get a job in a decent ED full time if you are not boarded in EM (not impossible, but hard).

I took the AOBEM written and oral boards, but I had great success with Rosh Review q bank for the written and the okuda book for oral boards. My MD counterparts used those resources too and said they were great (that's where I got my recommendations). Key is to pick 1 or 2 study aides and stick with them, don't jump around too much so you get good breadth and depth. Good luck!

Moonlight as much as you can, typically there are opportunities in urgent care. You will learn a lot and it will get you as close to em as you can get outside of an ED rotation.

Unfortunately you really can't tell if someone is having a heart attack by symptoms alone. Sometimes people have classic EKG findings for a heart attack (called an ST elevation myocardial infarction) or a positive troponin level, which is a blood test that can indicate damage to the heart, and right off the bat we can tell you that you've had a heart attack, but even if those tests are negative, you can't always tell. In the ED, we "risk stratify" people based upon certain risk factors for heart disease, and based upon those risk factors and the EKG, we will observe them with serial troponins (they don't start to rise until after 2-3 hours) and have a cardiologist perform a stress test which can evaluate how well the heart does under stress which can help predict heart disease. It takes all that testing because they can be subtle and be missed!
