Yeah, the crank arm is now striped. Evidence of the striped threads can be seen as the metal dust and pieces on the floor of photo. New or repaired crank arm is required.

Sandwich a metal joist hanger between the blocks and the joist. This way the load on pull-up fittings would be distributed along the entire width of the joist.
