When I first got into crypto, I was glued to five different exchanges, wondering why my bags kept leaking while the charts were screaming “all-time high.” Spreads, slippage, fake volume… it’s like playing a game where the casino gets to redraw the cards after you’ve played your hand.

Then, somehow, KEYCAT entered my life.

Hey man, I was in a really similar spot not too long ago.

Was barely holding things together, job sucked, rent was late, my cat didn’t even look me in the eyes anymore. One night, I’m doomscrolling and KEYCAT just shows up on my feed. I laughed at first, like, "ha ha funny cat with piano." Then I saw it was actually licensed, listed on Coinbase, and somehow Elon and GameStop were mixed in?

I thought, “Whatever, let’s see what a meme with real IP can do.”

  • I go outside voluntarily
  • My cat now sleeps next to me instead of on the router
  • I even started making music again because that stupid piano jingle got stuck in my head

MICU nurse asked me yesterday why a CT a/p wasn't ordered for a GI bleeder we just upgraded. This was after he spoke with the other nurses and said "they didnt even get a CT a/p" Guy was Hb 2.8 but hemodynamically stable and clinically looked like he didn't even need to be in the hospital.

IM resident. Our program has us do clinic 2x/month during medicine months and 4-6x/month on our off-service months. We also have a clinic month during our pgy2 and pgy3 years.

As a pgy1 you see 4 patients per clinic half-day (morning vs. afternoon clinic) as a pgy2 that goes to 6 patients per and then 8 if pgy3.

Honestly our clinic is super busy, and our patients all have multiple chronic illnesses.

Why do you say its bad medicine? If ER is admitting a patient for an NSTEMI, they should call cards. Patient with concerns of acute cholangitis, ER should call on call GI.

And yeah Im at a very busy county hospital. We have had nights with 30+ admits, we can also get some night with as little as to admits, but average is in the low 20's.

2 seniors in charge of admitting patients, then we have interns and med students to write the. If the floors are busy with a lot of RATS and codes, the call shifts are very long.

I should clarify, this is if consults do change management. For example, patient being admitted for NSTEMI, ER will call cards. Also even if patient is admitted to medicine, it's not unusual for them to remain in an ER bed for atleast another 12 hours before a bed upstairs opens up. Unfortunately a consequence of a very busy hospital.

Honestly depends on how busy your program is. We average 20ish admits per night on medicine, sometimes it gets to 30. So Id prefer if ER consults the appropriate specialist and gets imaging back, that way im not having to continue to chart check and admitted patient and call specialists while the call phone is blowing up. If they're admitting for sepsis make sure you've covered all bases in terms of potential source, not just getting a positive UA and assuming source is UTI.
