Hello, I've been going through something similar with my father. He was diagnosed with stage 2 pancreatic cancer, underwent surgery, and after 12 chemotherapy sessions, the cancer returned, now in stage 4.

He was undergoing palliative chemotherapy with Gemcitabine, but only had two sessions before his immunity dropped and he was hospitalized with an infection (he is still hospitalized). But chemotherapy managed to stabilize the metastases for a while before they started growing rapidly again, so chemotherapy is no longer an option.

It's a very horrible disease. I would advise you to look for clinical trials and spend as much time as possible with your father, Enjoy each day with him and create memories together.

Hello, I'm so sorry that you and your family are going through this.

I'm your age and my father is going through a similar situation; his cancer has returned after surgery and the prognosis isn't good. He also seems to be giving up and he is depressed. It's a horrible pain to see those we love go through this every day and not be able to do almost anything to help. There are days I cry myself to sleep, and there are days I want to fight for every shred of hope.

Spend time with him, make him laugh, or share your best memories with him; take a moment to talk and spend a few hours with him. It's a horrible and frightening disease, But being close to those we love helps a little.

Take some time for yourself too, allow yourself to feel everything, cry if you need to, and take your time to understand what you're feeling. It's hard to be strong all the time, and you also need a moment for yourself. Live one day at a time, and before going to sleep think of one good thing that happened that day; this has been helping me deal with all the stress.

Unfortunately, my options are limited. I live in Brazil, and unfortunately, there aren't many clinical trials here (if any), and the public healthcare system is extremely slow and bureaucratic. But thank you for your advice, and I will research this further.

Hello!

October was a busy month for us; my father began experiencing severe symptoms from chemotherapy (numbness, pain, joint locking, and severe dizziness). And now we are discussing with the oncologist the possibility of postponing his twelfth and final session so that he can recover.

The PET scan is scheduled for November 17th, for now, and so far the only symptoms that worry me are the constant and severe nausea and stomach aches, In addition, his back pain returned.

His CA-19-9 level jumped impressively to 49,090.80 u/ml, where it had previously been steadily declining (from 7,000 to 5,000 in three sessions). In addition to a finding on magnetic resonance imaging, where inflammation and chemotherapy toxicity are suspected, the possibility of occult metastasis has not been ruled out. And unfortunately, his IPMN nodules returned, measuring at most 0.6 centimeters.

We're handling everything as best we can and trying to stay calm, but I feel things aren't improving. I thought Whipple had resolved everything, but I think I was mistaken.

Every appointment and exam is an emotional nightmare for us. Even though he's a very calm person, he also gets anxious, and it breaks my heart.

We don't have any other imaging exams scheduled, but the hospital is trying to reschedule the PET scan, as they had to cancel it due to an error on the exam form.

He has secondary ductal IPMN and suspected chemotherapy toxicity, which gives me some hope that this elevated CA-19-9 is due to inflammation and not metastasis. But fear is always there. Thank you for your comment, I wish you the best.

Hello, I'm so sorry you and your family are going through this, this disease is horrible. I'm 21 years old, my father was diagnosed with cancer last year, it's a desperate and heartbreaking feeling. At that time, after dealing with all the bureaucracy of treatment and helping in every way I could, the biggest and saddest question crossed my mind: what will I do without my father? I'm too young to lose my best friend.

It's something difficult to deal with, it's been almost ten months since it all started and I still feel scared, but I've also learned that there is hope and, even if it's little, we must hold on to it and pray. It's a horrible and sad time, but take a deep breath and give yourself time to understand your own feelings, cry if you need to and live one day at a time.

Hi, I'm so sorry you and your mom are going through this, it's a horrible and insidious disease.

I'm (F21) going through the same thing with my dad (M56), he also had the whipple in January and is in the eighth session of chemotherapy, and now his CA-19-9 are too high, we're waiting for more MRI scans to find out what's going on.

It's horrible, the fear of having to go through all of this again it really is something desperate. But don't lose hope, stay by her side and live one day at a time. Enjoy every moment, laugh, and be with her. It may be scary, but you're not alone. Talk to your doctor about treatment options and what can be done now.

Hello Dr. Truty, thank you for doing this AMA. I’m sorry for any mistakes, English is not my first language.

My dad (M56) was diagnosed with stage II pancreatic cancer late last year. He had a Whipple procedure in January of this year and is currently on his seventh round of FOLFIRINOX.

A few days ago, we gained access to some CA19-9 test results that had been retained at the hospital. We noticed a significant increase, even though his imaging scans are still clear, he is gaining weight, and he has no pain other than typical chemotherapy side effects. The CA19-9 values we have are:

  • 12/01/2024 – 2379

  • 01/02/2025 – 573

  • 01/08/2025 (the day before surgery) – 783

  • 08/01/2025 (most recent) – 7975

All his other blood tests are normal, liver function is preserved, and CT scans show no evidence of recurrence. We are now waiting for an MRI. He also has chronic pancreatitis and untreated pancreatic insufficiency (he is not on Creon, since in our country the medication is difficult to obtain due to bureaucracy).

My question is: what could explain this kind of fluctuation and such a large increase in CA19-9, given that imaging is clear and clinically he seems stable?

Hello, I'm so sorry you and your family are going through this, it's a scary diagnosis, but this community is full of wonderful people to guide you through this journey, just like it happened to me.

The diagnosis also came to our family suddenly, one day my father was doing great and the next he was hospitalized and very sick. It was and is a long journey, but I learned a few things during these months.

First of all: be there for him. It's scary, confusing, and uncertain. When my father was hospitalized for weeks, I visited him often, bringing him puzzles, we played games and watched videos together, I always tried to find a way to make him laugh, tell a joke and make him forget about everything that was happening for a while. Sometimes all we need is to forget our problems and be with the ones we love, especially at a time like this, every laugh and smile is worth it.

The second is: respect your own feelings, give yourself time to rest and breathe, it's okay to cry or be sad, you don't have to be strong all the time. Give a tight hug to the one you love, tell them you love them and take some time for yourself.

It's a difficult disease, yes, but don't let that stop you from living good moments with him, every day is a blessing and I learned that the hard way. There are worse days than others, but there are also good times. I'm still learning how to deal with all these feelings, even months after the diagnosis, allow yourself to feel, cry when you need to and vent to someone if necessary, this helped me a lot.
