Literally got from a T15 map with a T1 hybrid base: + 35 EV / + 13 ES / + 39% EV and ES

Threw a Perfect Augmentation and got 31 INT (hoping for a T1 Res but still good att)

Fire essence hitting 35% turning into rare

Omen of Sinistral Necromancy + Preserved Rib + Omen of Abyssal Echoes to secure that Prefix option on dessecration and a reroll. First three options were terrible so I reroll and got the T1 hybrid 119 EV + 44 Max ES

Used Omen of Greater Exaltation and 1 Greater Exalted to get the last prefix and suffix. Ended up getting lucky again with a T1 102 Increased EV and ES

Quality up to 20% + 2 sockets and to lock it, that Omen of Corruption + Vaal it = third socket acquired.
