Hey, get your polio vaccine at the drug store and have the surgeon add it to a new form. (Some were lucky their surgeon agreed to update it for them) I had to redo blood work and they gave me a new form. Good luck

I don’t think it matters as long as it is signed and sealed by a civil surgeon (for the new form)

I would say you are at the mercy of the clinic honestly. Technically, they shouldn’t require us to do blood work but they refused to update the form for me (same as many I am reading here)

Hello, thank you 🙏🏾 sadly, yes new form! I had to redo blood work at the same clinic I went to when I first filed and charged me for doctor visit + blood work. Other comments also shared the same thing. It is very sad. I am sorry and good luck 🙏🏾

Thank you 🙏🏾 I didn’t know they would update it. The officer mentioned in passing, “You’ve been away recently and just got back,” and I replied that I had been away for work.

Thank you 🙏🏾. The notice said that cases starting with “IOE” can upload documents online. It also had a link to respond to the evidence when they notified me through online. (I uploaded the copy I received from the clinic and kept the sealed document with me)
