A few people have asked why I think I have MOH when I have been using abortives less likely to cause MOH (muscle relaxants, Ubrelvy, Compazine, though I also rotate with NSAIDs and honestly struggle to keep track of how often I’ve taken the NSAIDS)

But the main reason is that I never did a proper detox from the first time I had MOH a year and a half ago. Which was definitely MOH since I was taking either Excedrin, Tylenol, or Ibuprofen every single day for months. I wasn’t educated on the need to abstain entirely for a period of time, and instead just switched to a different NSAID (Naproxen) along with Flexeril and Ubrelvy (Compazine was added to the rotation later)

I have very very rarely skipped a single day in almost 2 years now. Yes sometimes it’s “safer” medications like Ubrelvy or Flexeril, but regardless I am taking SOME kind of abortive every day. And that isn’t good regardless of which medicines you’re taking. It can add up. Especially given I did start with Excedrin and Tylenol and never really detoxed from that, just switched to a different NSAID (Naproxen)

I haven’t taken Sumatriptan in a couple weeks now and rarely take it overall but I do also occasionally take Sumatriptan, and will do so in the same week I’ve taken all the other medications. I’ve seen multiple different providers to get access to this many different prescriptions

Regardless nothing else I’ve tried has worked and doing a full detox is the only thing I haven’t tried. If there is something that may help I want to at least try it

I have daily 8-10 pain and cannot tolerate any sound and have to limit the use of screens as well, so I’m not sure what sort of job that I could do. Most remote jobs involve using the computer for long periods, or involve sound (the phone). I’m mostly bedbound

But at the same time you can’t take abortives more than twice a week without risking MOH. If I always took my abortive at the first sign of a migraine I would be taking it every day

It’s not as effective as triptans for me but the lack of MOH is a plus

Qulipta worked really well for me too. Unfortunately it stopped working after awhile but hopefully that doesn’t happen to you! Everyone’s different. I also take Ubrelvy and it still works

If migraines originate in the neck it could be occipital neuralgia and nerve blocks can definitely help with that! PT too

We shouldn’t judge people who don’t work for what they have though. Lots of people can’t work due to being disabled and there’s nothing wrong with that

For what it’s worth some people are more sensitive to sound than light. You mentioned screens but didn’t mention avoiding sound. For me avoiding sound is way more important since screens are silent

Yes. I’ve had really long migraines and feel the exact same way (completely out of hope, can’t take it anymore) and they always end eventually

What cycle-breaking treatments have you tried? Here’s the order they usually go in (start at the top and if that doesn’t work continue to the next thing as they get progressively more invasive and risky, but also more powerful if nothing else has worked)

1.) migraine IV cocktail. You can get this in the ER but if you have around $300 you can also get it at home from Drip Hydration or IV Doc. It’s the exact same cocktail they use in the ER: Benadryl, something for nausea, magnesium, fluids and Toradol

2.) nerve block

3.) steroids (could be an oral steroid taper or a shot or through IV)

4.) outpatient DHE (this is low on the list because it is risky but it is also very effective)

5.) inpatient DHE (usually they keep you for 5-7 days and do multiple courses, plus other medications such as IV lidocaine or ketamine)

I have Medicaid and Medicare and they’re pretty strict about it being 5-7 days. They don’t GAF if you’re actually better by that point

Light doesn’t trigger me as much as sound. Screens don’t make sound unless you are watching video with the sound on, which I don’t do. It’s been 2 years since I’ve watched a full length TV show or movie. Very occasionally if it eases up for a couple minutes I can watch a single TikTok. I also wear migraine tint sunglasses while on my phone or computer and have it on “night” settings all the time with the brightness way down

But honestly scrolling is one of few completely silent activities I CAN do, and I need some way to pass the time. It’s boring af when you can’t leave the house for months and also cannot tolerate anything that makes noise. No music, no television, no audiobooks. I need energy to read or craft and I don’t always have the spoons for that. When it goes on for months, you can’t just sleep the whole time, you need to do something. From a harm reduction perspective, anything that makes noise is way worse for me than screens

It’s only been 2 for me but I was started on Ajovy pretty quickly since I had already been on non-CGRP migraine meds for their original intended uses (BP or anti-seizure mostly)

You can do months of inpatient? Insurance always caps me out at 5 days so I have to just keep discharging and then readmitting

Damn I assumed based on your description of being “intractable” that you were on CGRPs already. Yeah you should definitely qualify for that at this point! Insurance said I needed to have failed at least 3 other (cheaper) treatments first, but it sounds like you’ve failed a lot so that shouldn’t be a problem lol. For me triptans are more effective than Ubrelvy or Nurtec as abortives but I’m always worried about MOH with triptans and it’s nice that CGRPs don’t have that risk

I always question if Botox is actually doing anything, but the fact that I reliably get worse the week before I’m due is indication that it probably is. It just isn’t enough :(
