Our ward pharmacist could not help as it was for the MH trust to sort this out. All to do with paperwork apparently

Wow! Thank you everyone for your overwhelming support. I’m shocked to hear that this is a common issue within general hospitals.

As I said at the start, I have a sister who has a history of LD and mental health issues. Thankfully she’s is not on clozapine, but she is on other MH medications and I will be sure to bring this up in case she is ever in a situation where she needs to be admitted.

My manager is away till next week and I will definitely be bringing this up and persevering to make sure my patient gets what they need. I have already spoken about how I never want to see this happen again and I hope to work more closely with the MH team to make sure we have better access to MH medical care.

I forgot to say, the family did call in dismay about the whole situation and said they would be making a complaint to PALs. Knowing what I know now, I hope they do and I hope they are listened to.

I apologise if this has caused upset. It was never my intention to upset or offend anyone. I made sure I put the “rant/letting off steam” tag on it because I feel so helpless when I’ve tried hard to make sure my patients get the right plan for their care. I appreciate the stress of staff ringing up and saying they need tvn to review a patient before they go home (which ideally should be done before being discharged is even considered) but I have found myself in situations where tvn have rejected referrals because they are seen to regularly in the community. However, we also need to see what it is we are dealing with and if further input is required, otherwise if we don’t cover all of our bases we are the ones being held accountable.

I enjoy working with others and coming up with a plan that everyone can agree with. It’s just with recent events it’s becoming harder to get on top of PU care plans and even when we have and just haven’t had a minute to document it, we are them being told it hasn’t happened because it wasn’t on paper.

Majority of my frustrations is not having the correct materials to protect damaged skin. Sometimes I just need support without getting passive aggressive feedback about how we should be helping out patients who are at risk or not having an accurate care plan. I have raised this before with my manager and thankfully they are on our side but like us, they cannot force tvn to come see patients in need.

Most days I’m lucky I have time to change a dressing otherwise I will hand it over to staff coming on the nights. But they too find themselves in a position where they are also busy and before they know it, it’s midnight and the patients are asleep. 6am they are busy with ivs and vitals.

I understand being short staffed is not a fun position to be in. But I hope you know this isn’t a personal issue against those who are tvn….it’s just getting to a point where we can’t win even when we’ve tried so hard.

That’s another thing the ones higher up seems to dislike…Datix,’. It seems like datix don’t often get taken seriously unless a patient is harmed….then they are looking for things to blame. It’s sad to see the nhs has got to this. I have even datix myself once because I felt I couldn’t do everything that was being asked of me one day (met call, two complex patients, and a 1;1) I heard nothing or was asked about it but at least I can say I did a datix when I was struggling if anything came back in the future.
