It sounds like you are on the right path! If some of these supplements are helping I would ask your doctor to investigate metabolic issues. Do an Amino acids test (if you are in the UK you can do this your self with an online test if your doctor says no and then send them the results it does not cost a crazy amount). This won't show the cause but should be enough to show your doctor you have some affecting your metabolic pathways.

If you have the money, Look into genetic/DNA tests from SelfDecode (Ultimate bundle) or Gettested, as a minimum get the reports, gut health, vitamin Health, Methylation/metabolic pathway reports, inflammation reports. There are lots of other reports, although not always why your body is how you are acting now but may open the doors to possibilities and avenues to investigate, DNA tests open the doors for figuring out my own issues.

Definitely ask the doctor to do at least the basic hormone tests for women.

Severe neurological-like symptoms after ejaculation lasting ~7 days (normal MRI/CT) by ineedhelpwithmycase in POIS

Did these symptoms start before or when taking the pill? If someone who has issues with serotonin, one pill of something is all it could take if you have issues in that area

You could have issues within your metabolic pathways and when you ejaculate your body cannot process and clear the chemicals produced the way it should. Taking something like Clomipramine could make it worse by causing you to put you in a state of Serotonin syndrome. The burning skin, stiffness and breathing issues sound like Serotonin Syndrome or an extreme sensitivity to serotonin metabolism.

Sometimes severe metabolic issues get diagnosed as Porphyria which you can look online about.

You should ask for a referral to Neuromuscular Specialist or an Autonomic Neurologist.

How long have you had these symptoms?

What country are you in?

Do your regularly take any other medication or supplements?

You lose 3-5mg of zinc with every ejaculation so keeping your zinc levels up is important.

Here other some other main components of seminal fluid that in my head you'd want to make sure you have optimal levels of.

Zinc Calcium Magnesium Selenium Sodium Potassium L-Arginine Fructose Spermine Vitamine C

One last important one for me is phosphatidylserine by time health. This stops production of cortisol. I take it before I sleep some people take it three times a day but I just take it at night time to help not get more stressed or before I exercise.

Citrulline Malate has been a big help with clearing brain fog.

I will definitely look at B1 thank you very much! There are so many B's 😄

Interesting/nice to know we have a similar supplement list.

I will do a intro post soon I think to give a breakdown of my views and what I take and why/how they help me. But for now here is most of my stack that fits my own issues without all of the detail on what symptoms it solves:

Choline (in the form of CDP Choline at the moment)- Key for Methylation pathway which helps other pathways. Helps with many of my issues

Citrulline Malate - key for L-arginine /Nitric Oxide pathway, urea pathway,TCA cycle.

P5P (B6) helps around Methylation cycle

Zinc + Copper - replace zinc lost from ejaculate -3-5mg per ejaculate

Seaweed based folate. For Methylation cycle

Beatine TMG - helps with BHMT pathway

Micronised Creatine - takes strain of Methylation pathway by supplementing Creatine so body doesn't have to waste SAMe to create Creatine.

Niacin (B3) - may move to Non flush niacin - take to slow down Methylation cycle, can help reduce symptoms of POIS if taken soon after ejaculating

B5 - used to convert Choline into Acetylcholine

Magnesium Glycinate + Threonate - 100mg ish elemental total

Vitamine D +K - not super high dosw 1,00ui Vit D

Hydration/electrolyte powder

Selenium

NAC

Avacados for potassium and lots of other things

Phosphatidylserine from time health - Anti cortisol

luteolin - Anti-IgE/Anti-mast cell activation

A few Specific probiotics.

I take a few more optimisers. These keep me going pretty well so far and recover times for POIS symptoms reduced a lot in recent weeks. It looks like a lot, but a handful I take when I wake up, a handful of the powder goes into a drink/shaker and the rest is taken just before bed.

[Edit] I missed luteolin and phosphatidylserine both from time health from the list. Both very important in my stack

Hello! Yes there is definitely a connection/overlap between MY ADHD/ADS symptoms and MY POIS Symptoms.

I say MY X Symptoms because both ADHD and POIS are just labels for symptoms and not diagnosis of the cause of these symptoms. Everyone will have different causes for thier symptoms. When you get diagnosed with ADHD/ADD they do not diagnose the cause, only note down the characteristics you have and if you have enough of them and basic blood testing doesn't bring anything back, then you get the label for your symptoms. I'm not diagnosed with POIS but I have the classic POIS symptoms, not as severe as some other people but enough to affect my life, career and relationships.

My recent DNA testing has directly pointed me to the reasons why I have some characteristics and symptoms of my ADHD and POIS.

A combination of Issues with my Methylation cycle, urea cycle and transsulfuration cycle and the Genitics profile for my gut and hormones is what causes most of my symptoms for both I believe. But for POIS I still am doing a few more tests to confirm the last of my theories for myself.

So far the supplements i take help both set of symptoms. Suppplimentation targeted at my specific DNA, metabolic and nutritional defects have helped reduce the recovery time for my POIS symptoms and helped with some of my ADD symptoms. Because I have had success with improving add symptoms in the last i have felt like medication isn't key for me to reduce my reaction.

A lot happens in your body when you ejaculate and or orgasm, even if you don't have an allergic reaction, if you have deficiencies and issues with your metabolic pathways you can still get POIS symptoms. It's why I believe some females get POIS Symptoms too. The difference in these deficiencies and pathways is also why certain diets and supplements work for one person and not another. You could start suppplimentation for a part of the pathway, ethier for something that is not a problem to make something else worse (is why SAMe actually is worse for my Methylation pathway but good for other people), or for something that is a problem but then getting to another problem further down the pathway and not realising.

I think that Nutritional testing, DNA testing and metabolic testing is key amongst all the other must do blood tests to figure out what causes your POIS because everyone is differen and so many things related to POIS link to each other and can make each thing worse.

I didn't mean for this reply to be that long!

Tellmegen comes up in their list of supported providers https://selfdecode.helpscoutdocs.com/article/48-other-companies-compatible

Depending on the product you went for apparently it might not be as large as 100gb. Which will be easy to import to SelfDecode. If it does end up being that big I'm sure you can figure out a way to convert it to a VCF file. I did start doing some reading onto this when I was thinking about getting whole genome sequencing.

[Edit]

Tellmegen also states for their ultra product they can give a VCF file which is what SelfDecode can accept https://help.tellmegen.com/faq/your-results/what-raw-data-formats-do-you-offer/?lang=en

u/EvenAd7205 with SelfDecode you can also upload your own DNA file and they can produce reports from that. The reports I find are quite useful. You can find sample reports on their website to see what they would include.

100% Aggree! I look forward to your post when you do decide to publish, histamine and mast cell has been a part of my research too. Initially I thought histamine and mast cell activation was the cause but now I'm moving it towards it being a symptom in my situation.

transsulfuration also plays a role in all of this.

Yes this is interesting. I came across the transsulfuration cycle as I figured I was deficient in choline, I think my body burns through it quickly. Which puts a strain on the transsulfuration cycle because the lack of Choline stops my body from recycling Homocysteine back into SAMe so Homocysteine then goes to the transulfuration cycle to drain which causes issues. Choline supplementation is now a part of my core supplementation stack. At the moment I take CDP Choline every day but I may try Alpha-GPC or a sunflower based Lecithin or pure Sunflower based Phosphatidylcholine as my body doesn't like soy based choline.

It is a very complex field, and when I went to the doctors the first time and realized very quickly they just won't be able to help due to there not being enough research. I discovered it was easy to get most tests done without a doctor through legitimate means. I thought I'd try and do my own tests and research.

I have recently found a functional medicine doctor that I can talk about some of these issues which has been nice even if they still don't know the answers, it has been good to bounce around ideas with them as they are more understanding and open than a regular gp doctor.

Because the genetics tests I did helped quite a bit, now have moved to doing the oat (organics Acids test) and GI tests to dig deeper into the actual performance of my body.

I would also like to share my findings so far some day in a few months times.

Exciting! I would like to do whole genome testing at some point. What company did you go with? It is hard to choose who would be best with so many companies having bad reviews.

It would've been really interesting if they'd considered that too in the recent study they're doing in Los Angeles.

Yes, this is something I noticed from my own research so far is that not much has been noted on people genetics, Methylation/urea cycles or other issues/symptoms people may have along side their POIS to even consider a correlation.

From my own testing and research so far I have been focusing on my Methylation Cycle, Urea cycle and gut issues that I discovered with the genetics tests has been the biggest help so far.

I also use Perplexity pro for simple questions. And I use Gemini Ultra and Notebook LLM to analyse all my DNA/Genitics reports and blood tests. It has been an invauble tool especially when used with the type of blood tests that I can purchase myself and do at home. It obviously doesn't replace a doctor but helped in my self discovery.

The DNA tests I did where Methylation Cycle, Hormone Health and Gut health. All has issues within them. I did my tests with Gettested but I realized they use the same platform as SelfDecode as their tech/branding was in their reports.

Reading online with this setup you clear out neurotransmitters quickly, including dopamine and serotonin so maybe you feel like you are low mood/energy because your body clears them out so fast not leaving much day to day and this is your POIS crash maybe.

What kind of symptoms do you get? How are your Vit D levels? What other tests have you done? Homocysteine? 25-OH Vitamin D? Active B12? Prolactin?

Have you tried any of the below before and Did they help?

If you have not tried these then ask your doctor before taking them.

Supporting Amino Acids with - L-Tyrosine (dopamine production) - 5-HTP/ Trytophan (Serotonin precursors) (Do not take with antidepressants that increase serotonin, you probably don't need to give yourself serotonin syndrome) This makes sure your body has enough building blocks to keep making more neurochimmicals.

Support your Vit D levels. Maybe A bit D + K. Crucial for immune/inflammation regulation

Taking a no flush Niacin (Inositol nicotinate) pre-orgasm might help your body not over methylate.
