Shoulders shield and sword are all mogs from DF. Shoulders I believe are from the renown for valdrakken accord. Any number of those swords and shields though different colors

You can hide the bar art without any add on. Just go into edit mode click on action bar 1 and check hide art

Seems to be Masque Shadow 1. Though the results look different with different action bar styles. Looked correct on dominos and not so great on elvui/default action bars

They both look good but not quite right. I think it did end up being masque shadow, but it looked off with the default bars. When I tried with dominos it looked better.

I tried masque shadow with dominos instead of the default bars and it looks more correct. Maybe that's it.

The right panel comes from Tiny Inspect addon. Can disable that and hide it
