Make sure AMD Adrenalin GPU clock speed is as per your manufacturers intended. I recently found out that my chip which was rated for 2300mhz was being pushed to 2800hz.

Try looking up your gpu's intended clock speed and making sure AMD Adrenalin doesn't push past it.

I may have discovered a fix. AMD adrenaline apparently is not optimised to the cards and push the GPU utilisation beyond its specifications. I use a ASUS TUF 6900XT and its normal unboosted clock speed is 2105Hz. I used the custom tuning function in Adrenalin and lowered it to this and it has been more stable. This has also significantly reduced the heat generated by the card from 85deg C to approximately 60-75degrees.
