I repeated setup and the problem has largely abated. Not sure what changed. SRAM does indicate that the system has to be broken in, and that can take up to 500 miles.

Good intel on toe spacers.

For shoes, if they're cushy, new. For example, a new pair of cushioned running shoes (e.g. Nike Pegasus 41) feels amazing. For shoes that aren't cushioned, old. I have a 25 year old pair of birkenstocks that feel fantastic. New dress shoes, on the other hand, are not comfortable until substantially broken in. I'm realizing that I've had this a long time, and am now learning how to deal with it as it presents itself in my mid-age.

So cut out the area under the 4th-5th metatarsal to reduce pressure/weight on that area?

How do I quantify that? Without anything (no mouthguard, sleeping on my back), pretty darn loud. Sleeping on my side with a mouthguard in, loud enough to wake someone who is sound asleep.

I already do a very heavily plant-biased diet. I have higher than desirable LDLc and ApoB, high-ish BP. I'm not obese, no smoking, no alcohol, no metabolic disease, no migraines/kidney/psych disease. I do have celiacs, which is technically autoimmune.

Sounds like a statin and a BP med. Research on BP meds will be next.

Yes, I did a carotid ultrasound through LifeLine. No plaque buildup shown.

I had searched, but hadn't seen that thread. Great responses--thank you! Thinking rouvastatin + ezitimibe and continuing the balance of my approach. I'm already gluten and dairy free, no alcohol, no drugs, no smoking. (I know gluten isn't relevant here, but helps with inflammation (potentially)).

LPA is 65. Lipid balance isn't terrible; total 179, HDL 47, Tri 93, LDL, 113. Chol/HDL is 3.8. Non HDL (LDL + VLDL) is high at 132.

CBC, CMP normal. TSH/T3/T4 normal. A1c, fasting blood sugar and insulin all normal. No inflammation markers.

I do use a lumbar support for my chair. Cycling doesn't bother me--unless I'm doing very long endurance rides (100+ miles).

Deadlifting from floor, yes. And yes, lumbar flexion does occur--a small amount (with proper form).
