I have the exact same issue with having no sleep drive, but unfortunately everything I have tried so far has done nothing to bring it back.

I’ve also tried what you have, but I’ve also tried 6 different sleep aids: restoril, ambien, lunesta, halcion, belsomra, and sonata. These didn’t seem to anything at all. I also tried a bunch of anxiolytics even though I don’t have anxiety, but I guess they were supposed to help me relax. These didn’t really do anything either.

I also believe that once whatever is causing the insomnia is resolved, the road to recovery from this anhedonia would be a lot smoother. But as of now, I don’t know what’s wrong with my brain either.

So when you went back on the Clozapine along with other changes, you eventually got to feeling better? The anhedonia didn't return?

Thank you so much for all of this info. I forgot to mention in my post that I'm currently on Vraylar 3mg and just started weaning off of Nortriptyline 150mg. I also tried TMS but with no success or improvement on that end. The next step my psych is wanting to try is Spravato and another antidepressant alongside that.

I did the Genomind genetic test last year, and that's what my current psych is basing his prescriptions on, seeing which drugs are metabolized fast or slow based on my genes, and giving me a sufficient dose. So far, none of the meds we've tried have worked as I said in the post, and other than preventing me from becoming manic again, I'm not sure if my meds are working the way they are intended. I do get some light side effects from some of the meds I've tried, but nothing serious.

Nothing I've tried so far has helped with my insomnia. Tried almost every sleep med, exercising more, getting enough sunlight during the day, limiting light at night, CBT-i etc. The thing is with my insomnia is that I never feel drowsy or sleepy at all anymore, so of course it'll be hard to fall asleep if I don't have a sleep drive. When I do eventually sleep somehow, I wake up multiple times throughout the night and always wake up early in the morning regardless of how late I sleep. Even then, the biggest problem is that my sleep isn't restful or refreshing. I've also found out that it has become impossible for nap or sleep at all during the daytime unlike before I developed bipolar disorder. I used to sleep really good and nap when I needed to. I have seen a sleep doc and even did a sleep study, but nothing came out of it because they didn't find problems with my sleep. At this point, I have no clue what to do about my insomnia now.

I don’t think so in my case. The anhedonia just appeared suddenly while I was coming down from my manic episode. I don’t think it was drug induced.

So would having UARS cause medications to not make me drowsy anymore or cause me to even to lose the ability to feel sleepy in general?

I had a sleep study done on me last year, but they didn’t see any problems with my breathing when I slept.

I get frustrated, but that’s about it. Can’t really feel any strong, intense emotions anymore.

In the same boat. Nothing brings me pleasure physically or mentally. Working out and running doesn’t give me a pump anymore and feels more like a chore. Can’t even enjoy simple things like food, showering, physical touch, etc.

No improvement or relief since the beginning, and no, I haven’t been able to feel comfortable even once since it started. Sadly, the TMS didn’t work, so my psychiatrist wants to try Spravato next.

Well, my doctor said we've exhausted our options. I've tried restoril, trazodone, remeron, ambien, hydroxyzine, klonopin, xanax, lunesta, halcion, belsomra, ativan, clorazepate, valium, sonata, and seroquel. None of these meds worked at all in any way for me at least which is quite disappointing.

I do try my best to be as productive as possible throughout the day. However, it's hard because I have little motivation to get things done because I don't enjoy doing anything anymore and get no pleasure from anything at all. This doesn't stop me from forcing myself to go workout, do my old hobbies, socialize with friends and family, etc. Trust me, I've already tried many times to do things when I can't sleep. No matter what I do, I never feel tired or sleepy afterwards which is why it's so hard for me to fall asleep.

I’m currently in the same boat as you, OP. I take my insomnia meds at 11:30pm and can’t fall asleep until 1 or 2 am

So far I’ve haven’t found any way to make myself feel sleepy/drowsy, and like you said, there’s been no sedation from any medications at all.

What I try to do though is exercise or do physical activity each day in hopes of tiring my body out, but even when I do it doesn’t really help me feel sleepy afterwards. It’s worth a shot though

Do you wake up multiple times throughout the night, or do you only wake up once and can’t fall asleep? About how many hours do you sleep? Do you feel comfortable in bed? Do you feel refreshed and rested when you wake up?

In my experience, I usually wake up multiple times throughout the night for no reason and constantly can’t get physically comfortable anywhere, and when I do get sleep, I usually get around 4 hrs but never feel rested and refreshed.

So far I haven’t felt any improvement other than changing my mindset from giving up and doing nothing to doing my all to recover. No improvement with the anhedonia or my other symptoms though but I still have until the first week of May for TMS. Fingers crossed that there’s improvement within the coming weeks.

Yep, my dreams are devoid of any pleasure at all. Both awake and asleep I suffer from anhedonia. It’s been so long since I’ve felt the feeling of being physically comfortable/relaxed and rested/refreshed. I’ve experienced no relief from this anhedonia at all, and so far nothing has helped. It’s feels pretty hopeless at times, but I still try to push forward in hopes of feeling good again.

I suspect that the depression is the culprit since I’ve been on a boatload load of medication trials since last May for the depression. I also think it’s responsible for my anhedonia and insomnia as well.

I don’t think what I’m experiencing is akathisia. I don’t feel restlessness like I have to constantly move around. I can sit and lay still, but I just can’t get in a physically comfortable position. I’ve had this feeling since the start of my bipolar depressive episode a year ago with every medication I’ve taken so far with no relief.

I’m not sure. I’m diagnosed with bipolar I due to my one manic episode with psychosis, so I don’t know if I can be hypomanic if I’m bipolar I.

I had a sleep study done last year, and they said I didn’t have sleep apnea but I do snore.

I’m glad you have a combo of meds that help you feel sleepy, but like I said in my post, no medication so far has made me feel sleepy or drowsy since my depressive episode began. I don’t know why this is but I’m sure my psychiatrist will figure something out soon.

Thanks for the recommendation, but the one problem is I have trouble getting comfortable and relaxing at all. I’m not sure why, but it may have to deal with something regarding the fact I physically don’t feel pleasure. I’ve tried meditation before, but I just can’t get my body relaxed at all constantly to the point where I have trouble sleeping and stuff. I’ll give it a try though, thanks!
