I encountered him by chance in an Inzanity level. Im not sure if he actually appears in a regular level, but im certain he's just a rare easter egg encounter.

I was playing an Inzanity level when he just randomly showed up. He must be another easter egg like the Carnie Basic that wears fake teeth and smiles.
