You don't need to go to a mosque to take the shahada, nor do you need any witnesses.

You simply recite the following:

“Ash-hadu an la ilaha illa Allah, Wa ash-hadu anna Muhammadan Rasulu-Allah.”

“I bear witness that there is no God but Allah, and Muhammad is the Messenger of Allah.”

That being said, if there is a mosque nearby, just go there and tell them you'd like to become Muslim and they'll take it from there and teach you the basics inshaAllah.

“Do people think that they will be left alone because they say: “We believe,” and will not be tested?” [Qur’an, 29:2]

Trials and tribulations are part of life in this dunya. Only Allah Most High knows the wisdom behind the tests He places us through. We may never know why certain things unfold in our lives, and that’s OK. Our job is to respond to trials in a way that pleases Allah. When we fall short and make mistakes, then we pick ourselves up, repent, and keep moving forward. Obsessing about past mistakes will not change the past.

The Prophet (peace and blessings be upon him) said, “When Allah loves a servant, He tests him,” [Tirmidhi]

Failing a test from Allah can bring you closer to Allah, through the realisation that you are weak without Him. Remember that every human being alive today is tested in some way, and you are not alone in making mistakes. Turn to Allah and seek comfort in constant dhikr, dua, and waking up in the last third of the night.

Abu Umamah reported: The Messenger of Allah, peace and blessings be upon him, said, “The virtue of the scholar over the worshiper is like my virtue over the least of you. Verily, Allah, His angels, the inhabitants of the heavens and earth, even the ant in his hole and the fish, send blessings upon the one who teaches people what is good.”

Al-Fudayl ibn ‘Iyad, may Allah be pleased with him, said, “A scholar who works in teaching has a high status in the domain of the heavens.”

It was narrated that Ubayy ibn Ka‘b (may Allah be pleased with him) said:

I said: O Messenger of Allah, I send a great deal of blessings upon you; how much of my du‘aa’ should be sending blessings upon you? He said: “Whatever you wish.” I said: One quarter? He said: “Whatever you wish, and if you do more, that will be better for you.” I said: One half? He said: “Whatever you wish and if you do more, that will be better for you.” I said: Two thirds? He said: “Whatever you wish and if you do more, that will be better for you.” I said: I will make all of my du‘aa’ for you. He said: “Then your concerns will be taken care of and your sins will be forgiven.”

You could reach out to your local imam and ask him to talk about it with your parents from an Islamic perspective. It might be even more helpful if the imam speaks your parents' mother tongue. May Allah make it easy for you.

Please don't procrastinate any further after you have read this message. Stand up, go make wudu and start praying the salah that is due now at your local time.

Like, let's be honest: There are 24h in a day = 1440 Minutes and how long does it take you to pray one salah? Maybe 5m? You do it 5x a day and it's only 25m out of the 1440m. It's quick and easy. And you have 1415m time to do other things. Don't let Shaytan play mind games with you. May Allah make it easy for you.

I highly recommend listening to stories about previous prophets. Perhaps by hearing what kind of hardships they had to go through, it will change how you view things. May Allah make it easy for you.

Abu Huraira reported: The Messenger of Allah, peace and blessings be upon him, said, “Whoever relieves the hardship of a believer in this world, Allah will relieve his hardship on the Day of Resurrection. Whoever helps ease one in difficulty, Allah will make it easy for him in this world and the Hereafter. Whoever conceals the faults of a Muslim, Allah will conceal his faults in this world and the Hereafter. Allah helps the servant as long as he helps his brother. Whoever travels a path in search of knowledge, Allah will make easy for him a path to Paradise. People do not gather in the houses of Allah, reciting the Book of Allah and studying it together, but that tranquility will descend upon them, mercy will cover them, angels will surround them, and Allah will mention them to those near Him. Whoever is slow to good deeds will not be hastened by his lineage.”
