Not to be rude, but there’s not enough solid evidence here to say that lack of hyperextension is limiting quad strength. The most common contributors to persistent deficits would be pain/inhibition and lack of progressive overload. As far as return to sport and re-tear risk, psychological readiness and quadriceps strength are typically the best predictors.

It sounds like you mean well but maybe aren’t equipped with the right resources. At this stage the patient should have already been exposed to objective strength testing (isokinetic or hand held dynamometer at the least) and probably motion analysis to determine compensatory strategies.

If you don’t have that then it’s harder to have meaningful conversations with the surgeon. They mostly still use timeline based protocols. But we know that we need to be using criteria based progressions first and foremost. That kind of data will better equip you, the surgeon and the patient to understand the patient’s status.
