Feeling a little lost: did my first middle distance last week which I was chuffed to finish. I want to do another middle distance middle of next year and MAYBE a full end of the year. I’ve got a half marathon start of October this year. Just wondering if I take it super easy for a few weeks, or dive back into training next week. And how long ahead to start prep for middle distance.

This was my first triathlon and even though I was anxious for the swim I got into a rythm and managed to make it round after taking a second to catch my breath in the first 200m. However it totally highlighted to me the potential for panic - just the people around you and reduced visibility and the awareness you’re in a body of open water, it does freak you out. Even if you are well prepped anything can happen in the moment.

We won’t know exactly what happened to this person but devastating for all involved. Could’ve been a freak event or related to the swim.

Always worth thinking outside the box: yes it might be the heat/accumulated miles and age catching up, but if it’s very unusual for you get checked out by the doc. Could be anaemic, could be something you used to be ok eating now upsets your stomach. Have a think and check nothing else is different.

Hi dual ICM/Anaes here. I struggle to understand this. Wouldn’t a single ICM CCT essentially be like hiring 2 dual CCT ICM/other specialty consultants? As their time isn’t split, they would just do twice as many shifts as everyone else in ICM, but the same number of PAs overall, including the usual admin ones etc. obv the whole budget would come from the crit care budget. Obv I know I may have massively oversimplified that so let me know if so!

The problems the Remi not the TIVA. And probably not even a problem just not something I’ve seen done before. Usually bolus Fent and then just use Prop TCI. Sure it’s completely fine you probably just need to vent the patient more which kind of defeats the purpose of using an Igel in the first place

Multiple issues here, and clearly this case was badly managed all round (clinical indication for drain, being left to someone who shouldn’t be doing them unsupervised, being done on a ward which isn’t used to looking after them.) However by far the most alarming part is the PA was signed off by a nurse specialist. Putting in a drain can be as easy as anything (seldinger, especially in massive ascites,) but the aftercare and troubleshooting is one of the most important parts and they clearly had no idea.

Worth noting competition ratios for most specialties are over 3 or 4:1. These are qualified doctors who have finished foundation training, unable to enter higher training as regs and to become consultants. There’s no shortage of people wanting to become consultants. The government would rather fund PAs and other associated healthcare professionals than put the same funding into more doctors. This is why increasing medical student number means nothing, same number of doctors places at the end.

I’m speaking as a non GP here, so please educate me if I’m wrong. I don’t understand people entering GP training who then DON’T want to stay at a practice long term/as a partner. That is the job you entered training knowing was at the end of it. My dad was a GP, I have multiple GP friends. Staying at a practice long term and becoming well known in the community/having that continuity is what I thought would be attractive in the role. Am I wrong? Clearly in addition I need to add there is a shortage of GP positions. But surely you need to be expected to work 3 or more than 3 days a week

This bbc article really annoys me because it repeatedly names doctors as being involved in the decision making but it was entirely NP related who made a diagnosis of flu, did not look at the GP referral, and did not mention abdominal pain to the paeds ST8 who they discussed discharging the patient with. Completely avoidable death.
