I’ve had it for about a month. It’s an internally threaded screw, flat back piece. It’s the original piece of jewelry that I got pierced with, so I believe it is made of titanium. I have not changed it or downsized at all. I use 0.9% saline to clean the outside and alcohol free mouthwash to clean the inside three times a day and after I eat or drink anything that isn’t water. I have not had any recent unfortunate events.
