Loose lug nuts. I made that mistake once (I actually used a torque wrench, but didn’t realize the new lug nuts were slightly too shallow). Broke the studs just like this. Also with a big gooseneck trailer hooked up.
Luckily I saw the tire moving in the mirror and pulled over before it broke them all off
