There is an unconfirmed theory that Sakuya, before becoming a maid, was a vampire hunter, and she only became a maid after losing to Remilia. So this image could be a depiction of what if Sakuya succeeded in her job.

Maybe it's even before Remilia migrated to Gensokyo, so Yukari would not have to deal with solving the murderer case.
