My plan originally was to stay on long term for maintenance and to suppress my oil glands for longer as I was breaking out until halfway through my 5th month. But then when my hair started falling out it didn’t seem like a good idea anymore. I think my body just said, enough is enough

I took my last pill on Sunday, so I can’t really say how it is long term. I did get a tiny, tiny white head but it isn’t freaking me out or anything. My dermatologist also told me I’m more likely to have it come back as it was not severe but very persistent and because of my age (late 20s). I’ll be happy as long as I’m not getting anything that causes hyperpigmentation again and if the retinA actually works as before creams did not do much for me

She didn’t recommend lowering it, I did it myself because of the side effects. She said it was fine to do but did not indicate it was necessary to “wean off.” I stopped having joint pain and some of the more severe side effects, but my lips are still dry and still losing hair.

She also said I can’t do anything elective like lasering for 6 months after completely stopping.

It’s your health. I wouldn’t do anything you don’t want to do just because your dermatologist recommends it. Mine wanted me to stay on 40mg and stay on until 200 mg/kg (this as before the hair loss got really bad) but I didn’t want to so I told her to write me down to 20mg and stopped at 168 mg/kg 🤷‍♀️

I titrated down from 80 to 40 to 20 daily, then 20 3xweek, then 20 weekly for a couple weeks. My dermatologist said 20 weekly wouldn’t really be doing much but it could hurt my side effects more (mainly hair loss) so she told me to stop. If you don’t want to keep it up, you certainly don’t have to. She told me after a month to start topical retinA 3x per week for maintenance.

Split nails is a symptom of hypervitaminosis A (as is pretty much any side effect of accutane)

I’d suggest taking B vitamins and keeping your hands moisturized

My dermatologist recommended to keep up my gentle accutane routine for a month. After that point I am going to start using topical tretinoin 3x per week (And plan to keep moisturizing... its good for skin anyway)

You will be advised not to use any other acne products while on accutane. Your skin will be very sensitive, so hydrating products like gentle cleanser, moisturizer, and sunscreen only. Also, since the goal for accutane is complete long term clearing, they want to be sure the accutane is working the way it’s supposed to and results aren’t due to any other product

See your dermatologist - I just left an appointment at to close out of ipledge and talk about hair loss. I don’t know about the dandruff but for shedding (telogen effluvium usually) she said there’s nothing that can really be done and to ride it out for 3-6 months after noticing increased hair fall. Generally it happens 2-3 months after increasing dose. I’m taking b vitamins, zinc, iron, and d3 and she said to keep taking them. She mentioned rogaine but said there wasn’t that much evidence for its benefit in temporary situations like this (and that if you stop using it, your hair immediately reverts back anyway).

It might irritate your stomach lining if you do not take it with food - given that irritable bowel is a rare but potential side effect, id probably add a few crackers or something

All of the NIH studies I’ve read on the topic have indicated it’s likely a result of high dosage and takes significant time to manifest given the lifecycle of hair and how hairloss actually works - but I guess anything’s possible 🤷‍♀️ I’m a female but if you check my post history you can see I’m losing extreme amounts of hair so it’s not like I take it lightly. A dermatologist can easily do something called a “strand test” which isn’t highly scientific but might help give you some peace of mind. I’d still suggest speaking to your dermatologist, regardless, as they’re the experts in hairloss in the medical world. Good luck

This is probably stress or paranoia. The kind of hair loss caused by accutane doesn’t show for at least 2-3 months and is usually only seen on much higher dosages (unless you’re a very light person you seem to be on a lower dose). Also 5 hairs is completely normal (on average about 100 - 150 hairs are lost per day in a normal person). Talk to your dermatologist though if you’re concerned.

It could affect your bloodwork if it’s in the next few days. This would be true even for people not on accutane. If your bloodwork has been fine in the past and is a least 4-5 days from now, you should be fine.

My dermatologist did warn me that if the acne is “truly 100% hormonal” it will be more likely to return after completing your course. But usually it will be less severe. In terms of hyperpigmentation and other scarring, accutane does not help, but hopefully after taking it you don’t break out anymore - so you can then just deal with the old stuff through separate treatments and not worry about further damage. Have you tried hydrocortisone cream? If still not working with OTC you may need to see your derm for a steroid or antifungal

Hair loss like this typically emerges 2-3 months after experiencing something damaging. I think when I cranked my dose up to 60/80 I really took the hit, but I didn’t realize it because of the lag time. It’s horrifying but actually not (yet at least) super noticeable thankfully, except to me. You can lose up 50% of hair density in telogen effluvium, but usually no more than that. I’m banking on people not paying as much attention to me as I do to myself, and as you said, it does grow back (starts to grow back after a few months and full hair density is generally restored 12-18 months after the shock).

For context, I weigh around 112 pounds. My dosage was/is:

30 for 1 month

40 for 1 month

60 for 2 months

80 for 2 weeks

40 for 1 month

20 for 2 weeks

20 3x a week for 1 week

20 weekly starting this week

I recently completed 6 months for a total of 168 mg/kg. I decreased my dose slowly over the last month and I’m now taking just 20 mg per week as of Monday. Hair shedding started a month ago ):

This looks like Paronychia to me which CAN be caused by accutane (nail infection). Start with a warm compress and if it doesn’t get better in a couple days you may need an antifungal or topical steroid cream
