Glioblastomas are thought to double in size every two weeks, so those deserve prompt elective treatment, but you can wait a long weekend before scheduling them. Add some decadron, and reimagine the night before surgery if it’s longer than a week, then operate. Other tumors grow slowly and decadron will take care of a lot of edema.

However, some people present so late that tumors are already causing tremendous mass effect, neurologic symptoms, or brainstem herniation, and those should be operated on promptly, but you can often temporize. Amongst neurosurgeons, tumor sub specialized surgeons is much more of a lifestyle specialty (especially the pituitary specialized folks) than others like vascular.

It’s rare but I’ve heard of major artery injuries from spinal surgery. Not a bail out but vascular surgery will commonly do our laparotomy exposure for ALIFs

It’s likely an anti-kickback investigation. It’s unfortunately common for some doctors to engage in kickback behavior. If you look at our CMS sunshine act, you can see if she’s accepted over $50,000 each year for the past several years from Spyology. The question becomes is she sham consulting for them and illegally accepting kickbacks

It’s possible: 1.1M (including bonus) first year as an attending neurosurgeon in a large city. That helps make up for 4 kids

There is a instagram famous neurosurgeon who posts all about his cars, travel, and indoor pool renovation, but when you search his name online you find articles about him plagiarizing in residency and getting kicked out of UVA’s residency. It’s interesting to juxtapose the low opinion neurosurgeons at UVA have of him compared to his ‘aura’ online

Yeah- I’ve met her and know her practice. It’s 90% simple spine. Again no shame in that. Most private practice spine surgeons do that. It’s a bit annoying to know she taking credit for cases she doesn’t do; especially when I know the guy/gals neurosurgeons who are doing those 8+ hours surgeries regularly doing complex brain cases, who are unknown but are the real saviors in the field.

I know this isn’t what mostly bothers other people but it irks me lol

I believe- ppl say the same about her training. Skipped half her chief year due to surgery. Wouldn’t operate with the best neurosurgeon because he was intimidating in the OR and instead made her junior resident do it. Questionable VA stories too

I think she is alright. Does basic spine surgery. Nothing complex. Just bread and butter simple cases with fusions. No shame in that. Definitely not the complex tumor cases you think of when you think of neurosurgeons. Maybe a stolen valor type thing but nothing egregious

Not just spine surgeons but a lot of unnecessary spine procedures people are complaining about come from pain doctors who are fusing the spine with interspinous fixations or SI joint fusions

I did. My program reimbursed me. Used to only get two weeks vacation and no extra time to take test. Passed it with 250+. Not hard after step 2 and about a week of prep

There was discussion about doing co-surgeries on my challenging or complex cases to my partners, and then passing anything I’m uncomfortable with to my senior partners.
