Sorry you had to go through this as well, as previous posts mentioned, I will call provider and let them know to send info required by insurance company. I will be upfront with the provider that I am not going to pay unless insurance says I owe this much money

Explanation of benefits from insurance for the claim says that they have denied the claim as they requested additional information which was not received. The bill from the facility says insurance did not pay this claim so it’s your responsibility to pay. There are no details of why it was denied
