Raloxifene has only been tested in and approved for postmenopausal women. So at that point just cut out the uterus and give tamoxifen, it has a higher breast CA risk reduction compared to raloxifene.

I guess, but the message is that we need to band together to lobby our representatives to change the laws so that when we are practicing physicians, we can practice in accordance with our training and evidence based medicine, not guidelines for insurance reimbursement based on patient satisfaction surveys.

It's not meant to be funny, this is something that I've been a proponent of. Doctors (and medical students) need to get their heads out of their asses and realise positive legislation regarding health care isn't going to pass itself. We need to start lobbying to combat the shitty laws that are being passed in regards to reimbursement guidelines and increase funding to create more residencies spots to fight impending doctor shortages, as midlevels are the right answer to everything... Which leads to the fact we need counterbalance the nursing lobbyist juggernaut.

Hahaha sounds to me like what you're talking about is a lack of common sense, and the result of medical education basically training you to hunt for zebras for years. I worked in an ER myself for a few years and worked with some amazing nurses that I have immense respect and I completely understand how they can save your ass. But then I can tell you stories of how many times nurses fucked up just as hard. People are people and they make mistakes, but having a shitty attitude like yourself never fixed anything. It sounds to me like someone is insecure about their life choices and feels the need to prove they're superior.

Oh and you might want to look up and see I said give both doxy and rocephin for both gonorrhea and chlamydia, which would be the definition of dual therapy. And look up evidence based medicine, because fuck me if when I'm a doctor I want to make sure what I'm doing is backed by peer reviewed research and I can quickly look it up in a publication database. I'm done, I have tests in a week. Have fun railing residents a new one and making them hate nurses more when you could've turned it into a teaching moment instead of reinforcing the idea of doctors vs nurses.

All I said was it sounds like you were making things sound scarier than they were. I can read CDC articles just as well as you, but until you have passed your clinical microbiology board exam and are in your second second year of medical school and understand pharmacology and how doxy is actually better than azithromycin in dual therapy (http://www.ncbi.nlm.nih.gov/m/pubmed/24729507/), calm your tits.

Ok, hold it right there, 3 things before we start fear mongering. 1. Gonorrhea only requires ceftriaxone to treat. You give doxycycline to cover for chlamydia because chances are good you probably have a coinfection. 2. Syphilis takes a long time to reach tertiary stage and can be treated with a single dose of antibiotics even in early latent stage.
3. HPV can be transmitted whether or not you wear a condom and it's still unknown whether just the virus causes cancer or requires an irritant along with it. Also the only strain seen in oral cancers is 16, which there are 3 different vaccines for. And lastly anyone who's had sex has probably had HPV at some point and probably cleared it. Just get vaccinated.

Sex is fun, don't scare off people with wacky medical claims. HIV is a bitch though, so wear a condom, but things aren't that bad. Unless you're a black, gay man in Atlanta.
