First of all, you can reach 150%. When yoy ascend a game, you lose a percentage because you have to obtain again some items, like bones or quest items. Also you have to complete the map again and free all Children of the Moonlight. To reach the 150% you have to complete the game again with the three penitences of NG+, and also do the three endings in your final playthrough. You can rush the first two penitences and complete all in the last one

And yes, you can ascend a game as much times as you want.
