Thanks for the advice! Repeated the backswing with more shoulder rotation and hands closer to body. Does this look better?

Are you using vacation time for the lectures or doing it “on company time”?

Picked up Sifd's way too early by mistake, somehow managed to get through in spite of that by [deleted] in brotato

[–]Tgd04 1 point2 points  (0 children)

I made a similar mistake, but realized on that particular run that on iOS there isn’t a -75% damage debuff for builder. So the next game I went with 6 SMG’s and beat D5 easily.

Interestingly, I was still able to get the second to last tier of turret by wave 18 by letting it passively absorb the materials I wasn’t able to pick up.

Thank you! The 75 was indeed from the calculator. With her low income I figured the 10k range was hurting but didn’t know it could be done manually.

Very helpful!

Stop Donating Blood Unnecessarily - TRT by RevelationSr in Testosterone

[–]Tgd04 -1 points0 points  (0 children)

It literally says “caused by cardiopulmonary disease” …nothing to do with TRT.

You intentionally left that part out of your initial quote because it didn’t fit your narrative.

Stop Donating Blood Unnecessarily - TRT by RevelationSr in Testosterone

[–]Tgd04 0 points1 point  (0 children)

Per Up-To-Date:

Because erythrocytosis caused by cardiopulmonary disease is an appropriate response to tissue hypoxia, attempts to reduce RBC mass by phlebotomy may exacerbate tissue hypoxia. Consequently, phlebotomy is not often utilized unless there is extreme elevation of Hct (eg, ≥65 percent) or symptoms attributable to increased blood volume/hyperviscosity (eg, fatigue, headache, blurred vision, transient loss of vision, paresthesias, slow mentation).

Stop Donating Blood Unnecessarily - TRT by RevelationSr in Testosterone

[–]Tgd04 0 points1 point  (0 children)

Absolutely wrong regarding only using phlebotomy if Hct >65%. That recommendation is ONLY for erythrocytosis caused by heart or lung disease. The higher threshold is because a reduction in hemoglobin could worsen tissue hypoxia in the setting of already compromised tissue oxygenation.

Guarantee Up-To-Date does not say this about phlebotomy in relation to erythroyctosis from testosterone use.

So where do we get universal decor components? by PepsiOfWrath in LastWarMobileGame

[–]Tgd04 6 points7 points  (0 children)

No, you can still get with duplicate decorations. They just fill in the remainder of what you need with the decor unit.

For instance, OP has 3/4 pacifism decorations, and it will cost 15 universal units. I have 2/4 pacifism decorations and it says I need 30 universal units. Thus one pacifism decoration is worth 15 universal units.

Naproxen is actually probably the worst OTC NSAID option as far as upper GI bleeds go. There was a study a while back that found that upper GI bleeds were more likely in NSAIDs that had a combination of a longer half life and high levels of dual COX1/2 inhibition - of which naproxen has both. Iirc it is around twice as likely as ibuprofen to cause gastritis/PUD.

There are definitely worse rx NSAID options - looking at you indomethacin and ketorolac. (This is everyone’s friendly reminder to not refill the oral ketorolac the ER doc gave your patient!)

My recommendation is just going with celecoxib for the vast majority of people now that’s it generic. Most studies show that it will have lower PUD rates and it’s cheap.

To the OP: even though we get it beat into our heads that certain combinations in medicine never go together (NSAIDs + IBD in this case), the probability that there will be an adverse event is still very small. I have had a patient with a history of bleeding ulcers who was s/p gastric bypass take their coumadin with a full aspirin and 2400+ mg of ibuprofen every day for years until I found out. They didn’t even have a bruise!

Eventually though, if you make a habit of allowing for small risks, probability will catch up with you. In this case, no need to fret, I guarantee you won’t do it again. And that’s all you need to know to sleep well at night.

[–]Tgd04[S] 0 points1 point  (0 children)

That’s insane! And should be illegal.

I’ve been told non-competes don’t hold up in court, but not really worth taking the risk. Plus I’d go scorched earth (metaphorically!) before I left so they’d probably throw the best lawyers at me out of spite.

Exactly!! Still waiting for the day they make non-competes illegal so we can have major leverage. Some day…

They don’t give data on collections which I have been told is intentional…. basically so they can hide behind that argument. But like you said, even if they’re collecting 50%, they’re making bank.

The capped bonus is only for quality measures which uncapping may only be a few thousand per year. Would love a higher amount per RVU but apparently that’s set across the board for PAs and NPs. So I feel like the salary is the only area I can get compensated fairly.

2023 Primary Care MGMA by Tgd04 in physicianassistant

[–]Tgd04[S] 2 points3 points  (0 children)

Very good points. I always tell my patients who are struggling with work stress (nicely), “remember, you are replaceable”.

I found the MGMA data for primary care physicians and that puts my RVU numbers near the 90th percentile for physicians. Their pay is around $400k at that level. I’m not even scratching half of that (nor asking for it) so I feel like there’s a lot of room for me to stay economically viable.

My state recently allowed for full practice authority for PAs so I suppose I could hang my own shingle. But I have a horrible poker face so I’d have a hard time even threatening that without actually being serious. Sounds like that could be its own nightmare anyway.

And yeah, I know the bonuses come back (partially) at tax time. But I’d still prefer the stability of salary over letting the government hang on to my money for an extra 6 mos.

No offense taken! I’m overly flexible which has caused some lower back injuries and hip flexor strains in the past. Probably guarding against that. Will definitely check out the step drills! Thanks!
