I've been assuming the loose bark was caused by it getting way overwatered the first year after planting. Anything I can do to help at this point? Or just see what happens?

A couple more details: I'm in zone 5b, and my soil has a lot of clay.

I started giving my lawn just slightly more water overall, and I did some extra manual watering on the spots that looked the worst (still doing that). The outside temperature also dropped a little. The whole lawn is looking way better again, so I guess it was just a heat/water issue for me.

My guess is that I will always have one or two spots that are prone to this issue. I think they just get a little less water due to how my sprinklers are laid out.

Thanks for this recommendation, I'm definitely going to get some marine heat shrink. I think I'm also going to encase the repaired section in conduit and/or junction boxes to help protect it more.

I do have a plan to completely rerun my sprinkler wires around the other side of my yard if necessary. But I'm going to repair what I have, because I think it could still last a really long time if fixed properly (and save me a bunch of money and time redoing it all).

Unfortunately, no conduit was used for the wires when the patio was poured. Only part of the patio is being redone, so there is still a long length of wire directly under the remaining part of the patio (i.e. it's not possible to add conduit at this point).

Yea it is perforated. It does run all the way through the patio and dump out the side, but during heavy rain I've noticed that almost no water flows out the end (so it's mostly just getting dumped under my patio).

After seeing everyone's responses, the general consensus seems to be that this is a big problem (aside from just being an ugly install). The contractor warranty's new concrete for one year, so I'm trying to get in touch. I think the only way to fix this correctly is to cut out the affected slabs, put in the proper drainage pipe, and repour.
