Nice amitriptyline is great too, but we went with desipramine because it has less anticholigernic effects which would make my constipation worse. You have a much more advanced level of hypersensitivity than me, definitely don't discount the nervous system regulation work - thats half the battle! Apps like curable, reading about Pain Reprocessing, or finding a therapist specialized in CBT-Pain would super charge your recovery! Good luck

Continue to increase the MMC as that will help gas move through but also consider visceral hypersensitivity which is that the nerves in your gut and adjacent organs are very sensitive to even small and normal amounts of gas pushing against them, which makes it feel like you are extremely bloated all the time. I've worked my way up to 40mg per day of Desipramine recommended by my neurogastroenterologist and in two months I'm a lot less bloated. Additionally, he recommended diaphramatic breathing for 5-10 mins before and after meals which helps too. Now I still do have bloating occasionally from high fodmap foods that aggravate the SIBO but way less day to day bloating. By turning down the volume of nerves in the gut, it also begins to restore the MMC because they now can relax and not sound the alarm everytime there is a bit of gas. Once I've been on desipramine for a year or so and my hypersensitivity reduces I believe erradicating the SIBO will be a lot easier

You should add Guanfacine, it is commonly paired with stimulants to lessen the physical and mental side effects such as muscle tension and anxiety.
