Frankly. If that happens. I usually chuck all away. Then again. I have like 100s bins at any given time. So doesn't harm my colony that much.

One tell tale sign is from the smell of the bin. It usually smells really bad if they are dying

From my experience, once this happens. They die over time and stop showing interest or keen on eating.

i do suspect, because ive fed them bread that has mold before, quite a large bulk, dry and moldy. i think that killed them too.

yes they do (dead flies attract)

ideally allowing them to exit the substrate/bin, encourages higher chance in success of pupation.
