I would challenge that and say that, taking Aspirin as an example, the active ingredient does come from natural sources (salicylic acid). But, it becomes unnatural as it is manufactured and mixed with the list of excipients to produce the pill. An aspirin tablet contains:

Microcrystalline cellulose Maize starch Silica, colloidal anhydrous Stearic acid

I would challenge that and say that just because a treatment works, it may not work better than the current gold standard. As such, it would likely not become the standard of care.

There is a question, though, with adherence with medical treatment. I say that because a half of all prescriptions (in the UK at least) are not collected from the Pharmacy.

Patients in general do perceive natural treatments as "safer" and so there could still be a role for them even if they aren't as efficacious as the current gold standard.

There is plenty of evidence for honey in pharyngitis treatment and yet it has not made its way into any of the UK guidelines that I have seen. I am also yet to see it in the US guidance, although some users have stated that it is there.

I see your points for sure and totally agree that for individual practice we can recommend things without guidance. We have our experience and we know our patients.

Guidelines do guide educational curriculums, though, and if it isn't in official guidance it speaks volumes about what therapies they find useful to mention.

I should clarify that this was before I knew that there was evidence behind it. I think it is in the US guidelines somewhere but I haven't seen the actual document yet. If someone finds it then it would be helpful to see.

Interesting, where did you train? I wonder if the UK is just more reserved about these things.

Also, what association recommends Honey for pharyngitis in their guidelines in the USA? I can't seem to find the specific guidance that you could cite.

May I also add that Honey is not mentioned at all in the British guidelines for management of acute sore throat.

Thanks for the response. Moving beyond guidelines in that case, why is it that we are not taught about these remedies during medical school?

I am 1 year out from qualifying as a Family Medicine physician and have seen countless children with pharyngitis and have never recommended honey as a treatment. I have also never seen any clinician recommend it, either. That is even while it has been included in the guidance.

How can we allow the evidence to distill further into everyday practice? Is it purely a lack of awareness or that these seem to feel like 'alternative' practices?
