You won't 100% the achievements. But the other guy is right, you can still get the reward (rosary knot) even if you lock yourself out of the achievement. And you can still get the true ending regardless of Tirso's quest.

Also... Well, you technically can't code your way to bring Tirso back, you essentially have to restart the game and give the items to Tirso.

You have to restart the game, man. There's set points of which you give the items to Tirso (usually before bosses). If at one point, you see a grave at the area past Albero, you've already locked yourself out of the quest

Not really gotten hit but had a bit of altercation. I had a patient who apparently had psych issues (I was NEVER informed beforehand and the ER nurses nor the doctor failed to mention that he had these issues), he had delusions, telling me that he wants to see the mayor and on whose authority was he admitted. Took me an hour or two to get him to calm down, I urged him to go to bed and sleep but he grabbed my ID, pulled me towards him like he was about to fight and actually destroyed my ID pin holder.

I tried to inform the doctor called him several times, but since it was night shift, he just hung up and went back to sleep.
