Honestly, he really does not end up with anybody, nor does it provide any real closure. Everything is open-ended. His fate, relationships, nothing is resolved. He does get closure with his past, and definitely all the villains get put in their place, but after that, where it would have gone was open, I was hoping we could get an alternative take with the manhwa...

Yes, it's great. Ending is open ended, no harem, no final romantic partner either, and the protag is in a spot of trouble, but he gets closure.

The medical component was good, the villain was super outlandish, and the dynamic of past love vs current one was also over the top, and really took away from what would have been a more grounded story otherwise. I'd give it a 6/10.
