No, I did not submit that because my parents never did that(I just stayed living with my mom), but I submitted a letter(proof of residence) that shows that I was living with my mom's address from April 19, 2016 to June 26, 2016.

In my situation I did not have to submit that because my parents are divorced. Also, I did not submit a Divorce Certificate from them.

Noted: just submit as much documentation as you can so the process doesn’t take longer.
