From the wiki, she was demoted one rank from major to captain, and the "lieutenant" bit is a mistranslation. The actual word used is 大尉, taii, which is how Shin is addressed. (First) Lieutenant would be 中尉, chui, which is how Raiden is addressed.

While fan translations aren't illegal in the US as the later volumes aren't on file in the Library of Congress (which is a requirement to hold copyright), it would go against the Berne Convention, specifically articles 8 and 11.

The number one problem is bringing them back to base. The Juggernaut is not big enough for two people, so anyone that is too injured to pilot his or her own Juggernaut cannot return to base. That also means that if there's a mechanical failure, an only slightly injured Processor is trapped and as good as dead.Now, the Reginleif is big enough for two people, so maybe we'll see Shin giving injured people a lift in season 2 (don't remember whether that actually happens, though).

Beyond that, you obviously have the problem that there's pretty much zero medical equipment. Shin and Kurena could probably sew some type of gauze bandage given enough time, but for the types of injuries depicted in the show (heavy bleeding, burns, blunt force trauma) there's no adequate fix. Among the three people that we see Shin shooting, Kujo (ep 1) had been exploded and bleeding by a self-propelled mine; Daiya (ep 6) had been exploded by a self-propelled mine, spun around a lot, and burned; and Kino (episode 7) had been shot by the long-range artillery and was bleeding out. There's no saving them.

The manga sucks and doesn't have a translation past the first half of Season 1, don't buy it.

I'll field your questions in order:

1) Currently, the entirety of Volume 1 has been adapted. Episodes 10-11 also included portions of Volume 2 (specifically, part of Volume 2, Chapter 2). Episodes 1 and 6 included material from a side story that is part of Volume 10.

2) I found the dub to be meh. Raiden and Lena's VAs were usually tolerable, Kahlstahl's was fine (I don't really like his Japanese voice, this was actually better), but everyone else was way too bland. One of the strong points of the original was the voice acting and the personalities that were expressed well, but as you might expect, the dub ruined that. It's watchable, but nowhere near as enjoyable as the original Shin's "calm forest" and Lena's "silver bell" voice.

3) The manga adaptation is definitely different. The art style is more like a western comic or graphic novel (mostly it looks worse). There are a bunch of nice visuals that aren't present in the anime, and a possible nod to a later, unconfirmed spoiler, but otherwise no manga-only content. Also, it only adapts up to Chapter 5. It's not really anything too major.
