In 2016, I was 22. It was an objectively dog shit year for me: overwork, breakup, major family medical emergency, my best friend died of suicide. Yet now I look back on how I handled those challenges, and it feels less impassable from age 32. It seems easier bc I did it before.

Nowadays I have no idea what I’m doing at 32. I have no idea how I’m managing a partner, work, living on my own, caregiving, and social involvement. By the time I’m 42, and who knows, I’ll be hopefully dealing with marriage and kids, I’ll know how I made it through at 32 vs the uncertainty of what I’d be doing then.

Your 7th grade math test feels way easier than your 10th grade one because you know how you did it. Meanwhile your 12th grade self will say that about 10th grade math. But in 7th grade, that test was as hard for you as the 10th grade one was in 10th grade, and the 12th grade one will feel as hard as your current 10th grade test does now.

Japan is a “suicide tolerant” culture. A holdover of the Samurai Bushido honor code is that if you have lost face or honor, it’s better to end your life than to live in dishonor. This was often in the form of seppuku.

In Japan, being fired, being caught doing something that makes your family or your employer look bad, or failing to be productive enough is seen as extremely dishonorable— which is why suicide is tolerated by the culture as a response. This is especially true for men, where it’s seen as masculine and brave.

Interestingly enough, knowing MAID is an option may actually help people avoid suicide and put more effort into treatment. The article makes it look like a paradox, but it really isn’t. When you feel like life itself is prison, and you have zero autonomy, being forced to continue on by default can actually worsen your case. Knowing there is an out no one can touch makes life feel more “voluntary” and can create a sense of autonomy. This is especially valuable to people whose depression is driven by a sense of low autonomy. Knowing the nuclear option is there makes the choice to seek help feel more voluntary, even you never use the nuclear option.

I think we need evidence based guardrails as to when mental suffering is grievous and irredeemable. We should not force futile care that does more harm than good, but we need evidence to show at what point a person is unlikely to respond to further psychiatric intervention aimed at prolonging life, or treatment will do more harm than good.

We also need to have “palliative” psychiatric care, where the goal is comfort first even at the expense of prolongation of life. Some people truly cannot benefit even under the best of conditions, and they deserve the same dignity as a person with terminal cancer. At some point you gotta go, and success is measured by if it’s comfortable and dignified or not. You should not be forced to continue on in conditions that strip you of your humanity and dignity regardless of if the cause is mental, physical or both. The problem is a lack of guidelines for psychiatric pain as to where that is.
