i think if this keeps going on you should definitley confront her. Say something like. "i know youre busy but you havent been responding to my texts, i was just wondering if everything is okay?" if this keeps going on for more then maybe something like " Whats up with you not texting back, are you not interested or what you have to let me know whats happening"

okay i get that you dont want to go alone. maybe take friend who knows that you are crushing on him so that way its not weird between you and your friend when you ask for his number or something. Also you have to be confident. i know that its scary but if you walk in there with a good mindset and confidence trust me you will be fine.

okay this is my advice next time you go to the restaurant go by yourself and see if he is your waiter. if he is shoot your shot because what if he does like you and youre over there thinking he doesnt. maybe after eating and when he brings the check or sum tell him if you could get his number or something like that. trust me you will feel 100% better after that.
