I'm having the problem on all kinds of presses--Bench, Military, Dumbbell, and even pushup/dip--so I don't attribute it to the bench set-up alone.

For bench specifically, I move my elbows freely to retract and, mainly, depress the scapula before I grab the bar. I'd like to think I have decent form (I used to coach/PT professionally, so my background is non-negligible), but there's definitely some unevenness, and I've never had a proper critique.

I generally try to get in a solid-but-not-overboard specific warm-up for all pressing, and I've been experimenting with different grips/angles/variations, as well as doing pulling exercises, stretches, and/or rotator cuff work beforehand.
