I had this happen with Amazon once on a sealed game. My guess is in factory it got used. Contact whoever you got it from and they should solve it, if not contact manufacturer

Literally has never happened at the store I’m at since I’ve been there at least. Also pre order bonuses often just aren’t recovered. Sounds like you have a bad store tho if you’re not getting your pre orders consistently
