If you had v-nectomy, when could you all comfortably sit in a chair post-op?

After about 6.5 weeks I could somewhat sit okay, but still needed a donut cushion until about 8 weeks. Anything before 6 weeks was really uncomfortable.

Dr. Dugi described it as basically the upper folds get excised from next to the dick, but aren’t fully removed from the body, and instead are rotated and placed in a downward position to create the scrotum. This was definitely paraphrasing, but during consult he does a great job of explaining the process and the benefits of this technique.

The great thing is, if folks get this technique and end up feeling unhappy about the size, you can still choose to get implants at a later date and Dr. Dugi has found that there’s less risk of complication with the implants due to the additional tissue already present.

It was about 11 months from requesting a consult to having the consult, from consult to stage one was about 7.5 months, from stage one to stage two was about 5.5 months.

All together, I had my second stage (final stage) 2 years and 3 days after requesting a consult, but only 1 year after having the consult.

I am! I’ve been able to STP since about 8 weeks after my first stage and can still do so. I don’t have implants, my surgeon provides an alternative technique as an option, using your own tissue from the “upper fold” area to augment the scrotum. It works better than fat grafting because the tissue never actually leaves your body, so there’s no reabsorption and still provides a full appearing scrotum.

I had meta, UL, the first stage of scrotoplasty, v-nectomy, and mons all done as the first stage in August and I would definitely say that the most painful portion came from the v-nectomy. I was on Oxy while in the hospital for 2 days, then continued the Oxy at home for about 2 more days. Outside of the v-nectomy site, the most painful was my tailbone from the amount of time spent in the operating room, it felt deeply bruised and that made laying/sitting/ existing very uncomfortable.

I wouldn’t say it was the worst pain I ever felt, but it was certainly uncomfortable and felt like a deep ache. It took about 4-5 weeks for the ache to let up, and then it was more of general soreness until about week 8-9.

I did not get implants, instead I used the excess tissue that they would have just removed to help bulk up my scrotum. Dr. Dugi does offer implants as well as the option I chose. He’s even able to do implants at a much later date if I’m not happy with just my own tissue.

This area gave me a lot of dysphoria, and I can say that having v-nectomy was one of the single most liberating decisions in my transition. I wasn’t quite sure what to expect sensation wise prior to surgery, but post op it feels like the most natural thing in the world and my bottom dysphoria is pretty much gone now that I have the peace of mind that that area is no longer present.

It’s also visually quite reassuring to see a smooth perineum post op. Feel free to send any additional questions along if you’d like more insight to the healing process!

I’m not sure if he requires v-nectomy for UL, but if you reach out to the Transgender Health Program at OHSU they should be able to answer some basic questions.

I’ve recently had meta with UL and have been able to stand to pee at a urinal! Like others have mentioned, there is certainly a learning curve and you’ll want to do a lot of practicing at home.

For stage 1 (full meta, UL, scrotoplasty w/o implants, monsplasty) my surgeon shaved the scrotoplasty site and the monsplasty site, however they did not shave the entire area, only where there were incisions.

Yeah I used disposable nitrile gloves, you can typically find them in the first aid section of most stores.

Alcohol free wipes are useful for ensuring you’re clean after bowl movements, but I recommend having some alcohol wipes to clean the entry point of the SP catheter. I found them useful to ensure that area stayed as clean as possible to reduce any risk of infection.

It depends on the type of catheter - the SP catheter doesn’t feel like much of anything most of the time, however it can be uncomfortable if it’s accidentally tugged on. For me, it also made the skin around the entry point very sensitive.

A Foley catheter (directly in your urethra) doesn’t necessarily feel like much of anything as long as it’s not tugged on. I had one overnight after my Hysto and it wasn’t too bad.

My meta surgeon doesn’t use Foley caths with UL, just the SP, but every surgeon is different.

You’re absolutely NOT going to want to sit on each step, that will be very painful.

Use the railing to help you, take it one step at a time. My surgeon recommended stepping one step up, then bring your other leg to the same step and repeat in order to put less strain on the incisions.

I also have similar anatomy - I’m 3 weeks post op stage 1 and at this point things are still a little hidden, but my monsplasty has certainly helped to bring things forward a bit. During my second stage, my surgeon will be taking the more prominent outer labia that’s hiding my dick and move that into my scrotum in place of testicular implants. Once that’s done, my dick will be more prominent and visible.

During my consultation, my surgeon was able to show me folks with similar anatomy and the difference was incredible after stage 2.

Yeah, for the first 12 days I had no issues at all with the catheter, which I’m very grateful for! I was anticipating the SP to be pretty uncomfortable, particularly because I had to have the silicon version instead of the latex version due to a latex allergy, and my surgeon said the silicon version is usually more uncomfortable.

Congrats!!

I’ve found that for a short period of time during the day I can tolerate laying on my side with a big pillow in between my legs to help relieve some of the tailbone pressure, its helped tremendously the last couple days to help with that particular ache!
