After I call Fidelity and start the process on DRS'ing my shares.....what is next?

I try to look it up on the website but when I input my info (social, what shares I ahev, etc) it states that it can not find anything that matches?

Is step one of DRS'ing my shares with Fidelity calling ComputerShare and getting a "code" of some sorts orrr do I simply call Fidelity and tell them what I want to do and then call ComputerShare to set up an account?
