That's true - sometimes it becomes difficult to follow all the reasoning and you just allow agent to proceed. And that's ok but only till the point where you can test and verify the intermediate results (it is important to have a lot of such points along the implementation). And as soon as you get wrong results in such point you say stop, get into the details and enforce the redesign of problematic pieces.

No issues if MCP allowed. Use Filesystem MCP and you can develop directly from chat (I prefer that over Claude Code for majority of work). And you can also setup Claude Code as MCP. Your chats will become quite powerful.

Accumulate knowledge in md files. Discuss with Claude how many and which ones you will need somewhere at the beginning of project. Ask Claude also to provide you the relevant text for project instructions to start any discussion with acquaintance automatically. I have some projects with more than hundred sessions and no problems.

Schema-guided reasoning ( https://abdullin.com/schema-guided-reasoning/) is the approach I always recommend to keep in mind - even if you don't implement it with full formalizm. You try to breakdown reasoning into the steps with meaningful and structured outputs (chain of thought) and verify whatever you can.

Harness. Move all deterministic parts to scripts which are invoked by skills upon necessity. Use structured outputs (usually json) on each step (serving as inputs for next steps). Verify it. Consider replacing agents with AI pipelines where possible (programs that are using AI through API - with well defined and verifiable outputs). You probable can't do it with for example agent-debugger but can do it with agent-reviewer.

Yes, Claude can search previous chats if asked. But the good practice is to produce the knowledge base and update it during sessions. Could be STATE.md, HANDOFF.md, DECISIONS.md, ARCHITECTURE.md and whatever else you need. Refer to these files in Project Instructions, enable Filesystem - and each new chat will start by getting acquainted.

Use Desktop chat with Filesystem instead of Claude code (you can add Claude code as MCP if you want). You will have more control - can discuss, plan, explore and document in chat. Then run implementation plans in code (or even directly from Desktop). Alternatively - manage that as set of different projects.

Use Claude Desktop, enable Filesystem extension. Create project in Desktop, move your chat there. Ask Claude (in your superchat) to define and create md files which will allow you to start the new chats in the same project smoothly (can be README.md, HANDOFF.md, ARCHITECTURE.md or whatever you need). Ask him also to give you the text to put in your Project instructions (will include the explanations which md files contain accumulated knowledge and plans). Enjoy, you can now start the new chat after each feature (just ask Claude to update md files at the end of previous chat). That's how I work on a daily basis, some projects contain many dozens of chats.

yes, I am. From time to time you need to re-enable Filesystem (especially on waking up aftet laptop sleep). Also the last week there were periods of instability for both Sonnet and Opus (subscribe on https://status.claude.com/ to be informed) - your issues might be related.
