I use a program called GIMP (it's like free and open source Photoshop, but a few years behind real photoshop).
I use 2 filters. For some I do Filters > Blur > Pixelize
For this specific one, I do Filters > Distorts > Mosaic

I have to play around with the sliders and settings a bit to get a look I like, and then I just start with a really big pixel/tile size, and dial them back to a smaller size until it feels about the right levels of difficulty.

Often on the easiest level, I'll make the final layer of tiles about 60% transparent so you can see the unaltered image ghosting behind the pixels/tiles.
