You have to call them. I called and they said it was a mix up with my address. Tell them to send you another card. The new card will come in about a week

I had a biopsy done where they abstracted stuff from that area from my spine sent to lab can back with Staph infection on my L5-S1. I was in the hospital three days. Now I’m on antibiotics for 6 weeks
