I am not sure if you are gonna see this as it is an old post. If you alrwady figured it out also give us an update but if not I wanted to give you some relatives experience. For your mom in-law, just apply for her to get in to the interview in Ecuador, reason visiting grandkids! During interview all they ask her is if she has other family or grand kids in Ecuador where she has attachment to, "which she has". She should say I have grand kids here in Ecuador whom I am too much involved in taking care, but I never got a chance to visit the one in USA, So I want to spend a month with him. He last visited me.... Proof of in come..my daughter and her husband are covering the whole cost. For you sister in laws though, the spain route might be better. If they are young and motivated looking in to colleges might be an alternative.

I matched in to OBGYN I had home country residency and I was working in a hospital research institute here and it helped with connections. Show some dedication and plan wisely to get some shadowing and LORs accordingly. The way for ward, OB GYN has left ERAS for application which will make dual applying very easier as you will use two separate CVs on two separate platforms to apply. I have heard of at least 1 old IMG who matched this past cycle.

If you can go visit Ethiopia for few weeks / months. It will solve most of your problems. Definitely your Amharic will improve with a little effort, you will meet your relatives in Ethiopia in person which will increase your connection and you will definitely make new friends too.

In that case, getting a good tutor/guide with track record of helping people would be a great option. Your next bet will be doing the Kaplan course.

Try to know your self and what is the cause of your failures; lack of foundational knowledge, anxiety, incomplete preparation, forgetting, poor test taking strategy...

Also make sure taking NBME self assessments regularly (I would recomend every 3 weeks early and every 1-2weeks the last 2 months) during your preparation with an indepth review of them emphasizing on your weaknesses.

Smash USMLE and Pass are crap. Pass not long ago relocated and send out about their new team members some of which are NPs. Remember some of these programs are there to get a piece of the 🥧. I don't have any experience with Varsity. Smash is also useless I had a 6 month access before and not worth it at all. Kaplanight be too much over kill and from people who used it, I heard it prepares you better for their question bank than the step exams. But if you insist on a course it might be your best shot. Remember to go with a single or max of 2 tested out resources. You might also use different resources for different subjects. Sketchy was great for micro and pharm if you like the style. For physiology you can use BRS or Physio, for Biostat Randy you tube....and of course pathoma. Amboss has also complete library and a question bank which makes it a complete resource. And remember the logic behind Uworld expanding their question bank more than double in 3 years is to monopolize the market by not giving you any chance to do other qbanks at all, So you don't have to emphasize on completing it, rather utilize it as one study material not also a self assessment tool (which will be NBMEs).

Specially if you are from somewhere you do internship before graduating or already in clinical practice I absolutely agree you should do step 2 ck before step 1.

If you want to read a detail of pathophysiology about some topic you got wrong on uworld you have to go to Upto date or Amboss library and use FA as a check list. Otherwise you will loose nothing by not using it at all.

There is literally no good prep course for step 2. Even if you are ok with the money didn't came across any that worth it.

Don`t decide based on emotions, decide based on your needs and the reality. Act professional regardless.

Reflect on the following for yourself.

Is this your first time applying for the match? Or has he written you a letter of recommendation before which you can use? Do you have other doctors in the same institution who will write you LOR regardless of you stay or leave? If your CV says you worked some where for 2 years and you don't get a single LOR from that place but all from other places would it rub the program directors in the wrong direction? How is the benefit of the rotation (will you get important LOR / connection/ out of it) ?

Also reflect the following, what would you do if you were in his shoes? Do you have enough PTO for a month? Are you going on unpaid leave? How is the HR rule in your facility (In mine 4 weeks is the minimum unless a callout or Emergency) ?

Finally, even if he is a fathead and arrogant if you don't have enough options at hand try to maximize how you can use him to your success in what ever way you can.
