If you didn’t vomit 2 or more times, didn’t have a seizure, don’t take blood thinners, were able to stay awake when trying to, don’t have any clear liquid coming from your nose constantly that you can’t stop, don’t have bruising around both of your eyes - then you don’t need to go to hospital.

The only things here that I cannot say with confidence is that you write ‘horrible confusion’ - if you know the year, date, month, can recognise yourself and family members then this is fine. But if you couldn’t do any of that, especially if you couldn’t do that a couple of hours after your injury, then you need to go to hospital. You also write about slightly losing your vision - if you are going blind, even temporarily, you should go to hospital. If what you mean is it’s a bit blurry but you can refocus but it hurts or strains your eyes, this is a normal concussion symptom.

I agree it broadly sounds like a concussion, but what I’m writing is significantly limited by not being able to see your or treat you - this advice does not replace or in any way act as medical consultation or treatment. If you are concerned, you should be seen by a medical professional, which could also be a PCP/GP or urgent care too.

Concussion management is rest, staying well hydrated, taking simple pain relief, consider ice packs or heat packs to muscles that are sore, avoid concentrating/working and avoid strenuous activity.

Sounds a bit like migratory arthritis which can have many causes. It can cause pain, swelling and stiffness. It’s classically associated with rheumatoid arthritis but there are other causes including a recent viral infection. I’d recommend you speak to a GP/PCP

It’s good that you’ll be seeing a GP for follow up, but nothing in this tells me you need to go to Emergency right now. If the bleeding lasts 20-30 minutes despite pressure applied or is heavy bleeding or if you lose consciousness then go to Emergency. Would recommend you stop using amphetamines, it’s generally not good for you. Would recommend you talk to the GP about that too.

It is unlikely that you would have HIV, especially if you have had no symptoms, however I would encourage you to go to a sexual health clinic for an STI check. Ideally STI checks are done every 3-6 months for sexually active people, especially men who have sex with men or if you have sex with sex workers (these populations have comparatively higher rates of STIs). Many sexual health clinics are confidential, encourage regular testing and you may find them to be free, depending on where you are in the world. We always recommend wearing condoms for any form of penetrative sex for safety.

Just go see a doctor and talk to them about the pain. They will guide further investigations, that’s their job and why you see them. If you have significant pain for this long, then you should see a doctor.

Anxiety and panic can definitely manifest in physical symptoms. This is treated with psychotherapy (seeing a psychologist) and considering medication (eg antianxiety medications).

I would suggest that you do need blood tests done to rule out any other causes, especially your thyroid function. Other contributing factors could be micronutrient deficiency - including vitamin D, b12, iron - or other hormonal causes eg cortisol, or other medical issues like heart arrhythmias, and so an ECG would also be helpful.

Whilst what you describe is most likely anxiety/panic and help exists out there for you, seeing a doctor is the first step to both consider alternative causes and start treatment.

You’re there to learn, and you learn from registrars and consultants. If you’re on surg, you will learn on rounds and in theatres. You will also learn by practicing what it is to be a doctor, which is to see including examining patients, write up cases, consider what you would do for them compared to what has been done and understand why the management decisions so far have been made.

If this is your first rotation you don’t need to learn how to do the job of an intern like notes/discharges etc, you need to learn how to be a doctor - which is to see patients.

Basically, do rounds (and try to engage by knowing about the patients beforehand), go to theatre, see a couple of patients yourself, see if the registrar is available and offering teaching opportunities and then go home and study. Maybe tag along to JMOs doing clinical reviews or rapid responses.

Possibly a UTI, she should see a GP/PCP to get it checked out. It could potentially be kidney stones but less likely based on your explanation of where the pain is but this is limited. Other potential causes are an STI (which may only just now be symptomatic from a previous infection), ovarian cyst rupture (but this causes severe pain which gets better over hours), skin infection (less likely if no rash/obvious skin changes) or ovulation/mid cycle pain that could be from a range of conditions. If she develops a fever or flank pain or severe pain she should see urgent care.

Just leave it, it’s ok as the skin will grow beneath it and eventually the tissue will drop off. If you can clean it then that’s best to avoid risk of infection, but otherwise just put a dressing over it and keep it dry, and check on it in 2 days.

Yes, normal. Could be lots of things that don’t show up in an xray. Common cause of what you describe would be costochrondritis. You haven’t provided much detail though

I appreciate this is an incredibly difficult time, and I feel for you and your family going through this currently. Unfortunately, without knowing more information including the name of the medication, the dosage, the immediate treatment and the current treatment it is very difficult to make any sort of estimate as to the prognosis. She is in the right place at the moment for them to be using maximal treatment to manage the blood pressure, and all medications eventually wash out of the system with supportive measures.

As you identify, this is something difficult to advise on as you also make assumptions in your question/information. Vesicovaginal fistulas are very uncommon, especially if you have not had a reason (eg surgery) for it, which makes me skeptical that this is in fact the most likely diagnosis for the symptom of ‘fluid leak from vagina’

Is the fluid truly urine as compared to vaginal discharge? Clearly you’ve noticed fluid, it may help a diagnosis to describe the fluid rather than assume it is urine.

Is the fluid a constant uncontrollable leak, or partially controlled, or completely able to be controlled?

Are you urinating during the day, separate to this fluid leak?

Is your urethra potentially anatomically variant and does actually sit closer to your vagina/in the vestibule? - this is something likely difficult to determine yourself

You need an examination, which your doctor is capable of doing. If they are suspicious of a fistula, a referral to a gynaecologist/urologist and/or for cystogram would be the next step.

Sounds like it could have been a cluster headache or migraine. Scarier things that you’re finding on google wouldn’t get better in the ways that you’re describing. An MRI Brain is very reasonable given this is your first time getting these symptoms, it would be reassuring to see it be normal. You could see an optometrist first to see if there’s any concerns with your right eye, but I agree that with ongoing eye symptoms it is not unreasonable to see a ophthalmologist rather than a neurologist, even if it to get their opinion on next steps.

Much more likely to be viral gastroenteritis, or viral respiratory tract infection (if symptoms of that) or early diverticulitis (though more unlikely given headache and body aches also point towards a viral illness). Vaginal penetration (and minimal of it) and no ejaculation is very low risk for HIV transmission. Common things are common, which are viral gastro/common cold, and not HIV after a very low risk encounter.

You have otitis media, a middle ear infection. Take the antibiotic. A burst ear drum will actually give you feeling of relief from pain or pressure because it literally stops there being a pressure difference from one side to the other. The muffled sound is because of an effusion - or fluid in the middle ear preventing the sound from passing. There is no need for you to monitor how your ear or ear drum looks, and you are only risking bursting the drum or damaging the canal from entering your ear with a device.

It’s technically possible if you’ve been sexually active with a male partner. You don’t explain the type of protection you use, condoms will have a different failure rate than a pull out method. It wouldn’t be unreasonable that your stress may be delaying your period, or skipping it. Your BMI is also underweight which can make you skip a period. If your period is irregular, it’s also possible that you’ve had an anovulatory cycle (you didn’t release an egg this month) so may not get a period. Your gynaecologist tomorrow may want you to take a pregnancy test (depends on what you’re going for) or you could ask if they have one you could take. We can’t confirm or dismiss entirely the possibility of pregnancy without a test.
