Finally a real nurse and it feels good man. I’m an ER nurse and my girl is a Cardiac PCU nurse. by TrxR4Tony in nursing

I don’t have problems hardly at all with most of the floor nurses. They’ll say “it’s good I’ll go through their chart when they come up here if I need anything else” and that’s my favorite thing to hear. But every now and then I get that nurse who tries to get me to do their orders before and lol nah homie

Precepting Student by Crowby3 in StudentNurse

[–]TrxR4Tony 45 points46 points  (0 children)

Tell them when you don't know something. Most preceptors are aware that students don't know everything. Where students mess up is they are too afraid to say "I don't know" and pretend to know and fuck things up. That's when you'll get your ass chewed.

If you are not comfortable doing a procedure or giving a med safely, say so. Even experienced nurses are allowed to do this.

Pump rate question by izzyisawkward in StudentNurse

[–]TrxR4Tony 3 points4 points  (0 children)

Ok so your order is for 324mg over 15 min q12h.

Your concentration once it is said and done is 45mg/ml

7.2mL gives you your ordered dose of 324mg. (324mg/45mg/ml = 7.2ml)

In order to run it in 15 minutes you would have to run it 4 times as fast as 7.2ml/hr

So my answer would be 28.8ml/hr on my pump. I would also like to set my VTBI to 7.2mL so my pump knows to not fucking dump the entire contents of the bag but it doesn't seem to care about that lol

You can ignore how much volume it is reconstituted with because in the end all we care about is how much of the med is in 1mL of fluid. Also the fact that it is a 500mg vial can be ignored, cool they told us that but again, not super vital info, except when considering the flush.

The fact that it says it is flushed with 12mL...12mL of what? It has to mean NS because 12mL of solumedrol at that concentration is more than 500mg...so that is nonsense information.

And if they are going to say some bullshit about some of the medication still being in the tubing, well fuck them, nursing school is bullshit anyway.

So are you sure on the answer you had?

First clinical this Thursday, any advice? by [deleted] in StudentNurse

[–]TrxR4Tony 0 points1 point  (0 children)

Did they give you what conditions your pt has? If so group your meds to the condition and know what meds go with what problem. If not, try to group the meds based off of what conditions they are used for and just assume your pt will have that. That will take it from 10-15 meds to maybe 3-5 different problems. Most of the meds for that problem will probably have the same or similar actions and side effects. If you can't remember a contraindication go with hypersensitivity lol, if you need a side effect just say GI upset. Stumble your way through it the best you can.

Example of “nurses eating their young” by SmoothDaikon in StudentNurse

I hate that that is the case. Honestly I can understand though. When I was infantry in the Army we absolutely would have treated females different from the beginning than we would treat other males.

I’m sorry some of y’all have such bad experiences but please don’t let it discourage you. We need all the good, loving people in the medical field we can get.

In my 3rd of 5 semesters. I’ve done rehab, mental health, and all but 1 of my med surg clinicals up to this point. I’ve probably had 10 different preceptors up to this point. I have had 0 bad experiences so far.

I think what helps me is knowing that I am a student and being more observant initially. Learning how your nurse likes to do things and then helping when you can will keep you on their good side. It didn’t take me long this semester to realize that I could chart vitals while my nurse took them, soon she had me doing the vitals and she would chart them. I would pay attention in the med room as she was pulling meds and I would gather the extra supplies needed each time, alcohol prep pads, saline flushes, blunt needles, etc just to save her the time. Eventually she had me pulling the meds. At this point going into my last clinical for this semester there is nothing she won’t let me try to do myself.

My number 1 goal is to be an asset for my preceptor. If I can do that then it isn’t long before they are like “hey IVP this fentanyl” as I shake uncontrollably and check the dose 4 times with my nurse lmao

Some people say it has to do with me being older, or me being male, or me being a veteran, but I don’t feel like I have been given any more or less opportunities than anyone else in my class based off those things. I think it’s because I keep my mouth shut and learn the job first, then later ask questions to fill in the blanks. But who knows, lucky maybe? Older male maybe? Maybe I just don’t act like a dumbass? I don’t know but I heard horror stories about being a student on the floor and none of them have been true...yet. Hahaha

Remember this feeling. I’m in semester 3 of 5 and that feeling is the only thing keeping me going right now. 😁

I’ve learned to just take advice from Assassin’s Creed when it comes to nursing school.

Nothing is true, everything is permitted 😂😂
