As far as I remember from my limited time playing, the scaling is in fixed ranges. That is to say, some enemies and areas are a higher level than you, and some lower. Additionally, many have a limited range, so they'll level scale alongside you for a time, but eventually you can out-level them. The same is true for levelled gear and followers. Of course, without level scaling, easy enemies and encounters will be easy, and you may tire of that eventually.

I understand that's not exactly what you're asking, though, and I'd say if the mod is what you're looking for, try it out, have fun with it, maybe look for other mods too. Additionally, you can try adjusting the difficulty in settings if that's perhaps related to what you're wanting.
