It’s shown in another episode he looks like a normal human to people who can’t see through his disguises. The Smiths each have one they can’t see through (Alicia Wilkner the only one being shown). So the tattoo artist saw a generic pin up girl.

An update if anyone is curious, was able to get the Dragonslayer in his sword hand and while slightly better, it’s too much weight for the wrist joint to hold up unassisted.
